病变切除联合皮层热灼功能区治疗癫痫疗效分析

摘　　要：目的应用皮层热灼术,依据术前、中检测低功率电凝致痫区皮层横行纤维,治疗功能区顽固性癫痫,观察皮层热灼术疗效.方法 34例顽固性癫痫病人,手术切除病变脑组织,皮层脑电图辅助热灼功能及非功能区致痫灶.结果热灼后的部位呈白色条形轨迹.随访6～18个月,发作完全消失或偶有发作者为58.8%,总有效率为85.3%.术后8例发生一过性功能障碍,治疗后恢复.结论皮层热灼术作为辅助治疗功能区癫痫的手术方式疗效肯定,并具有安全、易操作等优点.Objective To investigate the effect of cortex thermo-coagulation on intractable epilepsy by coagulating the horizontal fibres in morbid area.Method 34 patients with intractable epilepsy were treated by resection of morbid brain tissues combined with cortex thermo-coagulation in functional and non-functional area,and their intra-operative electrocorticography（ECoG）was mornitored.Results White trace was formed after bipolar coagulation.The patients were followed up 6～18 months,no seizure was found in 58.8% patients,the total efficiency was 85.3%.8 patients were found temple dysfunction,and recovered after treatment.Conclusion The cortex thermo-coagulation is a effective way to treat intractable epilepsy,it is safe and easy to operate.
