体外受精—胚胎移植后异位妊娠58例分析

摘　　要：目的:探讨体外受精-胚胎移植(IVF-ET)后异位妊娠(EP)的高危因素。方法:对接受体外受精胚胎移植(IVF-ET)助孕后发生异位妊娠病例的助孕指征、助孕方式等进行回顾性分析。结果:2009年1月至2010年12月佛市妇幼保健院行体外受精胚胎移植(IVF-ET)2445个周期,1153个周期临床妊娠,发生率47.16%,异位妊娠58个周期,发生率为5.03%。年龄、促性腺激素总量、取卵数、移植胚胎个数及优质胚胎率与本次助孕后异位妊娠的发生无关(P>0.05)。49例患者助孕指征为输卵管因素和/或盆腔粘连,占84.48%。2种助孕方式(IVF-ET,ICSI)比较,总妊娠率差异无统计学意义(P>0.05),2种助孕方式异位妊娠率比较,差异亦无统计学意义(P>0.05)。结论:输卵管因素是异位妊娠发生的高危因素,与年龄、促性腺激素总量、取卵数、移植胚胎个数、优质胚胎率、助孕方式无关。Objective：To investigate the risk of ectopic pregnancy among women patients receiving in vitro fertilization and embryo transfer（IVF-ET）.Methods：the retrospective analysis of the indications and methods of assisted conception after the patients＇ receiving in IVF-ET.Results：From January 2009 to December 2010 in Foshan Maternal and Child Health Hospital,a total number of 2445 embryo transfers were performed.and 1153 clinical pregnancies were obtained,with the occurrence rate of 47.16,among which 58 were ectopic pregnancies,with the occurrence rate of 5.03.No significant differences were observed in women＇s ages,the total amount of gonadotropines,the number of harvested oocytes,the number of transfered embryos and the high quality embryo rates（P0.05）.Among all ectopic pregnancies,the assisted conception of 49 cases was tubal pathology and/or pelvic adherence（84.48%） 。The difeferences of clinical pregrancy rates between IVF-ET and ICSI were not significant（P0.05）,so is the case with the difeferences of ectopic pregrancy rates between IVF-ET and ICSI（P0.05）.Conclusion：Tubal disease and pelvic adhesions are high-risky factors of ectopic pregnancy while age,the total amount of gonadotropines,the number of harvested oocytes,the number of transfered embryos,the high quality embryo rates,methods of assisted conception are not concerned with ectopic pregrancy.
