摘 要:目的比较螺旋CT与MRI在强直性脊柱炎骶髂关节病变诊断中的应用价值。方法选择我院诊治的强直性脊柱炎患者35例,所有患者均行螺旋CT及MRI检查,对比分析两种检查方法对强直性脊柱炎患者骶髂关节病变的诊断情况。结果①螺旋CT共诊断0级病变4例,I级病变5例,Ⅱ级病变13例,Ⅲ级病变11例,Ⅳ级病变2例。②MRI图像中,35例患者均显示异常,其中Ⅰ级病变13例,Ⅱ级病变12例,Ⅲ级病变9例,Ⅳ级病变1例。MRI可以清晰显示关节滑膜、软骨及骨髓水肿等病变。③MRI对Ⅰ级强直性脊柱炎骶髂关节病变的诊断率为37.14%,明显高于CT的14.29%,两者间有统计学差异(P<0.05)。结论 MRI对早期强直性脊柱炎骶髂关节病变的显示明显优于CT,可用于其早期诊断。Objective To compare the value of spiral CT and MRI in the diagnosis of sacroiliac joint disease in ankylosing spondylitis. Methods Tirty-five ankylosing spondylitis patients were chosen, all patients underwent spiral CT and MRI examinations, the images were compared. Results ① Spiral CT showed 4 patients of 0 grade, 5 patients of I grade, 13 patients of Ⅱ grade, 11 patients of 11I grade, 2 patients of IV grade. ②Tirty-five patients showed abnormalities in MRI images, including 13 patients of I grade, 12 patients of Ⅱ grade, 9 patients of 11I grade, 1 patients of IV grade. MRI can clearly show the synovium, cartilage and bone marrow edema lesions. ③The rate of grade I sacroiliac joint disease in MRI images were 37.14%, much higher than that of CT ( 14.29% ), there were significant differences between in the two groups (P 〈 0.05 ). Conclusion In the early stage of sacroiliac joint disease in ankylosing spondylitis, MRI is better than spiral CT, it can be used for its early diagnosis.
