调冲止痛方治疗原发性痛经的临床疗效观察

摘　　要：目的探讨自拟方调冲止痛方治疗原发性痛经患者的临床疗效、前列腺素PGF2α水平、子宫血流动力学改变,探讨该方治疗痛经的作用机理。方法原发性痛经患者100例,随机分为治疗组60例与对照组40例,中药组服调冲止痛方(温肾活血解痉)中药,芬必得组于痛经时服用2天,连续3个月经周期。比较两组治疗前后临床症状(痛经程度评分、复发时间)、前列腺素PGF2α水平、子宫血流动力学改变(子宫动脉搏动指数和阻力指数),探讨中药作用机理。结果①治疗组有效率为88.3%,对照组有效率为85.0%,治疗组疗效优于对照组(P<0.05)。②两组治疗前后痛经症状评分差异显著(P<0.05)。③两组治疗前后子宫血流状态得到改善,两组间有显著性差异(P<0.05)。④两组痛经患者血清PGF2α在治疗前后有非常显著性差异(P<0.01)。结论调冲止痛方能改善子宫血流动力学,降低前列腺素水平,明显改善患者痛经症状且具有复发率低、不良反应少、对月经无影响的优点。Objective To observe the clinical therapeutic effects and the changes of PGF2,uterine blood flow in treating primary dysmenorrhea by Chinese Herbs.Also investigate the mechanism of action of the Chinese Herbs.Methods The patients were set to be Chinese medicine therapy group and control group.The Chinese medicine therapy group treated with chinese herbs that warming the kidney,activating blood circulation and spasmolysis,while the control group treated with Ibuprofen Sustained Release Capsules while dysmenorrhea occures.After 3 months treatment,compare curative effect,PGF2 and uterine blood flow in the two groups.Results In the Chinese medicine therapy group,the total effective rate was 88.3%;while in the control group,the total effective rate was 85.0%,curative effect of the Chinese medicine therapy group surpassed the control group,and had significant difference.uterine blood flow in the therapy group was better than the control group,and had significant difference.PGF2α level of therapy group was lower than the control group,and had very significant difference.Conclusions Chinese Herbs can improve uterine blood flow,decrease PGF2 level,ameliorate dysmenorrhea symptom.
