活动期类风湿关节炎患者血清游离κ、λ轻链与DAS28相关性分析

摘　　要：目的 探讨活动期类风湿关节炎（rheumatoid arthritis,RA）血清免疫球蛋白游离κ、λ轻链的变化,并与DAS28进行相关性分析.方法 选择活动期和稳定期患者各40例,并设40名健康者为正常对照;采用速率散射比浊法检测血清免疫球蛋白游离κ、λ轻链;采用SPSS 11.5 软件进行相关性等统计学分析,并用ROC曲线评价血清游离κ、λ轻链对活动期RA 的鉴别作用.结果 活动期RA患者血清免疫球蛋白游离轻链水平显著高于稳定期患者和健康者（P＜0.05）,且血清κ、λ轻链与DAS28呈正相关关系.κ轻链诊断活动期RA的AUC为 0.871;λ轻链诊断活动期RA的AUC为0.781.结论 活动期RA患者血清κ、λ轻链水平显著增高且与DAS28密切相关,可作为判断RA疾病严重程度和病情活动程度的重要指标.Objective To explore the changes of the serum κ, λ light chain and analyse the correlation in active rheumatoid arthritis（RA）.Methods Clinical data of eighty RA patient were elected by carrying out retrospective analysis,then on the basis of DAS accumulated points,they were divided into active and inactive group respectively,who were contrasted with 40 health adults.We applied rate nephelometer measures to detect serum κ, λ light chain.The correlations were observed between DAS28 and K, k light chain by SPSS 11.5.ROC was used to evaluate the role of the serum κ, λ light chain in the diagnosis of active rheumatoid arthritis. Results There were marked increase in serum κ, λ light chain and significant correlations between the serum κ, λ light chain and DAS28 in active rheumatoid arthritis.The area under curve of receiver operating characteristic（ROCAUC） were 0.871 and 0.781. Conclusion The serum κ, λ light chain are significantly increased in patients with active RA,and it may be a useful parameter for monitoring disease active of RA and therapeutic target.
