摘 要:目的探讨脊髓脊膜膨出超早期显微外科治疗的疗效。方法 2004年6月~2009年1月对26例脊髓脊膜膨出超早期(出生后72 h内)在显微镜下行神经松解、还纳、硬膜修补术。结果手术时间(120±36)min,出血(25±11)ml。按Kirollos术中松解分级评定方法:23例达到Ⅰ级,2例达到Ⅱ级,1例为Ⅲ级。26例术后随访3~12个月,平均8.4月:治愈11例,好转12例,无变化3例,无加重。结论严格掌握入选标准,遵循微创原则,显微外科超早期治疗脊髓脊膜膨出可以取得满意疗效。Objective To explore the efficacy of ultra-early stage microsurgical treatment for myelomeningocele(MMC).Methods A total of 26 patients with MMC received microsurgery in our hospital from June 2004 to January 2009.During the operation,following the principle of minimally invasion,under a microscope,the nerves were loosed,the spinal cords were returned back and the spinal canal and dures were repaired in an ultra-early stage(72 h).Results The procedures were completed in a mean of(120±36) minutes with a mean blood loss of(25±11) ml.According to Kirollos Assessment,23 patients achieved grade Ⅰ,2 were grade Ⅱ,and 1 was grade Ⅲ.The 26 patients were followed up for 3-12 months with a mean of 8.4 months,during which 11 were shown being cured,12 were improved,and 3 had no response. Conclusion By strictly following the surgical indications and principle of minimally invasion,ultra-early stage microsurgery is an effective treatment for MMC.
