超声对短期胰岛素泵治疗改善2型糖尿病血管功能的临床观察

摘　　要：目的利用血管回声跟踪技术(echo-tracking,ET)测定肱动脉血流介导的血管舒张功能(flow-mediated vasodilation,FMD)评价2型糖尿病(type 2 diabetes mellitus,T2DM)患者经2周胰岛素泵(CSII)强化治疗后,血管内皮依赖性舒张功能改善情况。方法将例不同病程T2DM患者分为3组,病程<1年为DM1组(38例)、1～5年为DM2组(34例)、>5年为DM3组(32例)。另选体检中心健康志愿者为NC组(30例),对照组年龄、性别相匹配。DM患者及30例正常人均应用高分辨超声探头检测右侧肱动脉FMD。T2DM患者在应用胰岛素泵强化治疗2周后重复检测。结果 (1)与NC组比较,DM组患者FPG、2hPG、TG、血管弹性参数β、Ep、PWVβ明显增高,FMD、AC明显减低;(2)胰岛素泵强化治疗2周后,DM组FMD、AC测值均增高,血管弹性参数也有改变,但改善程度不同,DM1组FMD、血管弹性参数改变最显著(P<0.05)。结论病程小于1年的初诊T2DM患者胰岛素泵强化治疗后内皮功能改善最显著。Objective To investigate the improvements on flow-mediated dilatation（FMD） by Echo-tracking technology in type 2 diabetes mellitus patients with different courses after two-week insulin pump therapy. Methods Type 2 diabetes mellitus patients were divided into 3 subgroups according to courses：38 patients in DM1 （less then 1 year） ,34 patients in DM2（1 to 5 years） and 32 patients in DM3 （6 to 10 years）. Meanwhile, 30 age-and-sex-matched healthy people in health examination were selected（NC group）. All research subjects accepted high solution ultrasound detection on arteriae brachialis for FMD. After insulin pump intensive therapy for two weeks ,all the indexes were re- examined in DM group. Results （1）Compared with the NC group,the DM group had significant higher level of FPG, 2hPG, TG and blood vessel elasticity parameters such as β, Ep, PWV β but lower levels of FMD and AC. （2）After two- week insulin pump intensive therapy,the DM group had higher level of FMD and AC. Blood vessel elasticity parameters had different improvement. The most significant subgroup is DM1 group （P〈0.05）. Conclusions Insulin pump inten- sive treatment could improve endothelial function better in type 2 diabetes mullitus patients with courses less than 1 year.
