超声影像评价下肢静脉解剖变异的初步研究和临床意义

摘　　要：目的应用超声影像技术研究正常人群下肢静脉解剖变异的情况,了解静脉变异的常见类型、声像特征和发生概率。方法 923例下肢静脉无器质性病变的普通患者行双下肢静脉连续超声扫查,双侧同名静脉对照观察。探查各静脉节段是否存在变异,如果发现血管变异,则仔细观察变异血管的情况,储存变异血管的静态、动态图像并做相应记录。结果 923例受检者有268例共517条下肢发现静脉解剖变异。下肢静脉解剖变异总发生率为29%(按例数计算)和28%(按下肢数量计算)。最常见的解剖变异类型为静脉数量变异,约占总变异量的91%,而静脉位置和解剖关系的变异则相对少见,约占总变异量的9%;最常见的解剖变异亚型为双股浅静脉(26%)、其次为双大隐静脉(8%),双股深静脉(7%)及双腘静脉(5%)。结论下肢静脉变异在正常人群中是较普遍存在的,而不是个别、偶然的现象。Objective To study the types, character of ultrasonic images and distribution of variations of bilateral lower limb venous anatomy by ultrasound. Methods 923 patients with non-organic disease of lower limb veins were scanned by color Doppler ultrasonography. Bilateral isonym veins were comparatively observed. Care was taken to ex- plore each segment of the veins and recorded whether any variations of the vein had occurred. Results Variations in the anatomy of the lower limb venous system were detected in 268 subjects （517 limbs） out of 923 subjects. The total incidence rate of variations in lower limb venous anatomy was 29 % （according to the number of subjects） and 28%（according to the number of lower limbs）. Quantitative variability of the lower limb veins was the most common anatomic variation, accounting 91% of the total variations. Variation of the venous position and anatomic relationship was scarce, accounting 9 % of the total variations. The major subtype was duplicated superficial femoral vein （26%）. The next was duplicated great saphenous vein （8 %）, followed by duplicated deep femoral vein （7% ） and duplicated popliteal veins （5 %）. Conclusions Variations in lower limb venous anatomy are not individual and incidental phenomena but common in normal population.
