高血压患者冠状静脉窦CT影像学初步研究

摘　　要：目的应用多层螺旋CT初步评价高血压病患者冠状静脉形态学的变化情况。方法随机选取门诊及住院行多层螺旋CT冠状动脉成像检查的患者100例,其中高血压病组及对照组各50例;高血压组女24例,平均年龄62.3±9.7岁(35-78岁);对照组女22例,年龄58.6±7.7岁(平均44-77岁)。采用美国GE公司的16层螺旋CT机,将原始横断面CT图像传送到SUN图像工作站,进行曲面重建和容积再现(Volumerendering)等方法进行图像重建。观察冠状静脉窦的长度、冠状静脉窦口的直径。对上述测量指标进行成组设计两样本均数t检验统计学分析,统计学显著性差异定义为p<0.05,高度显著性差异定义为p<0.01。结果高血压病组与非高血压冠状静脉窦比较:a.高血压病组:冠状静脉窦口平均直径为10.5±3.2(4.2-20.8)mm;冠状静脉窦心中静脉汇入处平均直径为9.8±2.2(5.5-15.8)mm;冠状静脉窦左房斜静脉汇入处平均直径为8.6±1.9(5.2-14.4)mm;冠状静脉窦平均长度35.6±9.0(13-61)mm;b.非高血压组:冠状静脉窦口平均直径为10.6±3.5(4.6-17.9)mm;冠状静脉窦心中静脉汇入处平均直径为8.9±2.2(3.8-11.8)mm;冠状静脉窦左房斜静脉汇入处平均直径为7.9±1.4(5.2-10.4)mm;平均长度34.8±7.5(20.9-49)mm。高血压病组冠状静脉窦在心中静脉汇入处及左房斜静脉汇入处直径增大,具有统计学显著性差异。结论高血压病会使冠状静脉窦在心中静脉汇入处及左房斜静脉汇入处的直径增大。Objective To explore the morphology changes of the coronary venous system in hypertensive patients by using multi-slice spiral CT（MSCT）.Methods Randomly selected 100 cases who accept multirow CT coronary angiography,including hypertensive patients 50 cases（24 cases are femal）,average age 62.3± 9.7（35-78 years old） and healthy adults 50 cases（22 cases are male）,average age58.6±7.7（44-77 years old）.We studied by contrast enhanced MSCT which retrospective ECG-reconstructions.The length,diameter and morphology of hypertensive patients＇coronary sinus were measured from both 3-dimersional volume rendered images and 2-dimersional cured planar images,and compared with the results in healthy adults 50 cases who accept multi-row CT coronary angiography.The main purpose was to observe the morphology change of the coronary venous system.A probability value p0.05 was considered statistically significant,p0.01 was considered quite statistically significant.Results The diameter of coronary sinus in hypertension patients：The average diameter of the coronary sinus orifice was 10.5.±32（4.2-20.8）mm.The total length of the coronary sinus was 35.6±9.0（13-61）mm.The coronary sinus average diameter in the middle cardiac vein was 9.8±2.2（5.5-15.8）mm.The coronary sinus average diameter in the left atrial oblique vein was 8.6±1.9（5.2-14.4）mm.The diameter of coronary sinus and its branches in healthy adults：The average diameter of the coronary sinus orifice was 10.6±3.5（4.6-17.9）mm.The total length of the coronary sinus was 34.8±7.5（13-61）mm;The coronary sinus average diameter in the middle cardiac vein was 8.9±2.2（3.8-11.8）mm;The coronary sinus average diameter in the left atrial oblique vein was 7.9±1.4（5.2-10.4）mm.The difference in the hypertensive patients and healthy adults：There were significant differences in the coronary sinus average diameter in the middle cardiac vein and in the left atrial oblique vein.Conclusion Hypertension makes the coronary sinus average di
