华东地区颅脑创伤患者住院费用及相关因素分析

摘　　要：目的对华东地区颅脑创伤住院患者的住院费用及相关因素进行分析,掌握颅脑创伤住院患者的疾病负担。方法整群抽取华东地区80家协作医院2004年全年收治的颅脑创伤住院患者的数据,用SPSS 13.0统计软件包分析。结果颅脑创伤患者住院费用最高达380 000元,平均14201元。日住院费用平均1 042元。13 007例患者的总费用为184 719 497元。25～44岁组的花费最多,占43.2%。多元线性模型显示住院天数、省份、性别、年龄、居住状态、手术、入院GCS评分、受伤机制、受伤地点、损伤类型、费用支付方式、病房类型、预后及医院级别都可能是影响颅脑创伤患者住院费用的重要影响因素。结论颅脑创伤患者住院费用高,经济负担重,应重点防治中青年男性患者因交通伤所致的颅脑创伤,以减轻颅脑创伤给社会及家庭所带来的沉重负担。Objective To analyze the hospitalization costs and the related factors of patients with traumatic brain injuries（TBI） in East China and to analyze the direct economic burden of TBI.Methods All sampling of the TBI patients from 80 hospitals in East China in 2004 were clustered and were analyzed using SPSS 13.0 software.Results The highest total cost for hospitalized TBI patient reached 380 000 RMB（average 14 201 RMB）.The average daily cost was 1 042 RMB.The total cost of 13 007 patients was 184 719 497 RMB.Patients aged from 25 to 44 cost the most,accounting for 43.2%.Multivariate analyses showed that length of stay（LOS）,province,age,sex,residence status,surgery,Glasgow coma scale（GCS）,injury mechanism,incident site,injury type,payment type,ward type,glasgow outcome scale（GOS） and hospital level were likely the main determinants of hospitalization cost.Conclusion The hospitalization costs and economic burden of TBI patients are heavy.The prevention of TBI for those between 25 and 44 years old male people should be strengthened,especially for the prevention and control of the traffic accidents to minimize the burden of TBI.
