HER2表达与乳腺癌超声造影表现的相关性研究

摘　　要：目的 探讨乳腺癌HER2表达与超声造影表现的相关性.方法 133例乳腺癌患者术前行超声造影检查,应用时间-强度曲线分析软件测定定量参数、时间-强度曲线类型以及肿块增强模式；术后行免疫组化法（IHC）检测HER2表达,分析HER2表达与超声造影表现的相关性.结果 HER2表达与峰值时间负相关（P＜0.05）,与强度差值、增强强度指数、上升斜率正相关（P＜0.05）,与TIC曲线类型有相关性（P＜0.05）,与峰值强度、始增时间、增强模式无相关性（P＞0.05）.HER2表达“3＋”组与阴性组间在始增时间、峰值时间、峰值强度、强度差值、增强强度指数及上升斜率间差异有统计学意义（P＜0.05）；“＋”组与“3＋”组在始增时间、峰值强度、强度差值、增强强度指数间差异有统计学意义（P＜0.05）；其余组间超声造影各定量参数差异无统计学意义（P＞0.05）.“2＋”组与“3＋”组HER2乳腺癌时间-曲线类型差异无统计学意义（P＞0.05）,而在其他组间差异有统计学意义（P＜0.05）；各组间增强模式差异有统计学意义（P＞0.01）.结论 不同级别HER2表达的乳腺癌超声造影表现存在差别,表现在始增时间、峰值时间、峰值强度、强度差值、增强强度指数及上升斜率等定量指标上,同时也表现在肿瘤增强模式以及时间-强度曲线类型等方面.Objective To explore the correlation between expression of HER2 and contrast-enhanced ultrasonography（CEUS） in breast cancer.Methods CEUS was performed on 133 cases of breast cancer patients before surgery,and time-intensity curve analysis software was used to measure the quantitative parameters,the time-intensity curve type and tumor enhancement mode.HER2 expression was measured by immunohistochemistry after operation.Statistical analysis was performed to explore the correlation between HER2 expression and the performance of ultrasound imaging.Results The peak time was negatively correlated to HER2 expression of histological section after operation（ P 〈0.05）.Enhancement intensityindex,increase slope rate of the curve,intensity difference were positively correlated to HER2 expression （P〈 0.05）.TIC curve type was correlated to HER2 expression（ P 〈0.05）.The arrived time,peak intensity,enhanced mode were not correlated to HER2 expression （ P 〉0.05）.Conclusions Parameters of CEUS are correlated to HER2 expression in breast cancer.
