中年人群血清生化指标与颈动脉硬化的关系

摘　　要：目的:分析中年人群颈动脉硬化(CAS)与血清生化指标的关系,发现中年人CAS的危险因素。方法:465例中年体检者经颈动脉超声检查,分为颈动脉硬化组(CAS组)和正常组。收集入选者的临床情况,并抽取空腹肘静脉血检测生化指标。包括:空腹血糖、血脂成分、肾功能、白蛋白、总胆红索。结果:465例受检者全部进入结果分析。CAS组的空腹血糖、尿酸、总胆固醇、低密度脂蛋白胆固醇高于正常组(P<0.01),载脂蛋白A1/载脂蛋白B100比值低于正常组(P<0.05)。而甘油三酯、高密度脂蛋白胆固醇、肌酐、白蛋白、总胆红素在2组间差异无统计学意义(P>0.05)。Logistic回归分析显示:年龄、尿酸、血糖、总胆固醇进入回归方程,其OR值分别为1.140(1.076～1.208),1.005(1.000～1.009),1.395(1.066～1.827)和1.461(1.027～2.079)。结论:年龄、尿酸、空腹血糖和总胆固醇水平升高可能是中年人群CAS的独立危险因素。Objective： To investigate the association of blood biochemical indexes and carotid artery arteriosclerosis（CAS）in middle-aged population undergoing physical examination, explore the possible risk of carotid artery arteriosclerosis. Method： The ultrasonography was performed in the carotid arteries of the 465 middle-aged people,by which,they were divided into CAS group （ n =290） and control group （ n = 175） according to the results of ultrasonography. In addition, their biochemical parameters such as blood glucose, lipids, uric acid, albumin and total bilirubin were examined respectively. Results： Totally, 465subjects were included the final analysis without any drop-out. The serum level of blood glucose, uric acid, total cholesterol, low density lipoprotein in subjects with CAS were significantely higher than those in control group（ P 〈0.05）. Ratio of apolipoprotein A-1/apolipoprotein B100 was obviously lower（ P 〈0.05）. There was no significant difference in triglyceride, high density lipoprotein, albumin, creatinine and total bilirubin（ P 〈0.05）. Logistic regression analysis showed that age, blood uric acid, blood glucose and total cholesterol were involved in the regression equation. The ORs of age, blood uric acid, glucose and total cholesterol for CAS were 1. 140（1. 076-1. 208） ,1. 005（1. 000-1. 009） ,1. 395（1. 066-1. 827）and 1. 461（1. 027- 2. 079）respectively. Conclusion： Age, high level of blood uric acid, blood glucose and total cholesterol could be independent risk factors for carotid artery arteriosclerosis in middle-aged population.
