钢针撬拨反弹器固定及切开复位内固定配合中医药治疗跟骨骨折疗效比较

摘　　要：目的:探讨透视下经皮钢针撬拨手法复位反弹器外固定及切开复位内固定术配合中医药治疗跟骨骨折的疗效比较。方法:我科自2006年5月至2010年10月收治跟骨骨折52例(62足),分别采用透视下经皮钢针撬拨手法复位反弹器外固定及切开复位内固定手术治疗,术后按照中医辨证理论体系,分期选择中医内治、中药熏洗、中医按摩等康复治疗,术后随访半年至4年,平均1.8年。结果:透视下经皮钢针撬拨手法复位反弹器外固定66%,良28%,可6%,切开复位内固定手术组优74%,良22%,可4%,两组间差异采用t检验,按检验水准=0.05,P>0.05,两种方法治疗效果无明显差异。结论:跟骨骨折采用透视下经皮钢针撬拨手法复位反弹器外固定及切开复位内固定手术治疗,效果无明显差异,术者应根据患者年龄,骨折类型,工作强度,经济条件及自身对各种治疗方法的熟练程度综合考虑选用合适的治疗方案。Objective： To discuss the comparative effect of treating calcaneal fractures by needle poking a rebound device and open reduction and internal fixation combine with TCM. Methods： Our department from May 2006 to October 2010 calcaneal fractures treated 52 patients （62 feet）, were used under fluoroscopic percutaneous needle poking a rebound manipulative reduction and external fixation devices and open reduction internal fixation surgery, postoperative according to the theoretical system of TCM, Chinese medicine in treatment selection staging, herbal fumigation, Chinese massage rehabilitation therapy, patients were followed up six months to four years, an average of 1.8 years. Results： Perspective percutaneous needle poking a rebound manipulative reduction and external fixation devices 66%, good in 28%, medium in 6%, open reduction and internal fixation group were excellent in 74%, good in 22%, medium in 4%, the difference between the two groups t-test, according to test level = 0.05, P〉0.05, two methods no significant difference in treatment effect. Conclusions： The perspective of calcaneal fractures using percutaneous needle poking a rebound manipulative reduction and external fixation devices and open reduction and internal fixation, no significant difference in effect, the surgeon should be based on patient age, fracture type, intensity of work, economic conditions and their treatment of a variety of proficiency in considering the appropriate choice of treatment.
