摘 要:目的探讨胆囊胆固醇性息肉实时超声造影表现。方法对45个经手术病理证实的胆囊胆固醇性息肉的超声影像资料进行回顾性分析。常规超声观察息肉的大小、形态、回声、血流参数。超声造影观察息肉的增强时相及增强方式。结果 45个胆固醇息肉,直径5~23mm,平均(9.47±4.48)mm。直径≥10mm的息肉100%(17/17)测及血流,<10mm的息肉38.9%(11/28)测及血流。流速范围0.11~0.28m/s,阻力指数0.51~0.65。实时超声造影显示45个息肉均有增强,41个(91.1%,41/45)胆囊病灶增强快于肝实质,4个(8.9%,4/45)与肝实质同步增强,而减退方式上,21个(46.7%,21/45)减退慢于肝实质,22个(48.9%,22/45)与肝实质同步减退,2个(4.4%,2/45)快于肝实质,表现为稍低回声。增强方式上,蜂窝样增强38个(84.4%,38/45),整体增强7个(15.6%,7/45)。结论超声造影可提高胆固醇性息肉的血流显示率,慢于或同步于肝实质减退及蜂窝样增强的表现对其诊断有重要意义。Objective To investigate the significant performance of gallbladder cholesterol polyps by conventional and real-time contrast-enhanced ultrasound (CEUS). Methods The data of ultrasound image in the 45 patients with gallbladder cholesterol polyps were retrospectively analyzed. The size, shape, inner echo and blood parameters were observed by conventional ultrasound. The enhanced phase and pattern of the polyps were observed by CEUS. Results Of all the 45 polys with the maximum diameter ranging from 5 mm to 23 mm (average 9.47±4.48 mm), blood flow signals were detected in 17 polyps with the diameter larger than 10 mm (17/17,100%) and 11 cases smaller than 10 mm (38.9%, 11/28). The velocity in the polyps ranged from 0.11 to 0.28 m/s and RI from 0.51 to 0.65. All the cases were enhanced (45/45,100%) by CEUS. Compared with adjacent liver parenchyma, 91.1% (41/45) of the polyps were quickly enhanced and 8.9% (4/45) were synchronously enhanced, 46.7% (21/45) were slow, 48.9% (22/45) were synchronous and 4.4% (2/45) quickly decreased. Honeycomb-like enhancement appeared in 38 polyps (38/45, 84.4%) and overall enhancement in 7 polyps (7/45, 15.6%). Conclusion CEUS could slowly improve the flow signals displaying rate of gallbladder cholesterol polyps and synchronously decreased pattern compared with adjacent liver parenchyma. The thin honeycomb-like enhancement has important value in differential disease diagnosis.
