血尿酸水平与慢性充血性心衰患者心功能及高血压关系的研究

摘　　要：目的:研究血尿酸水平与慢性充血性心衰(CHF)患者心功能及高血压的关系。方法:选择我院收住的NYHA心功能分级Ⅱ～Ⅳ级的CHF患者192例,根据心功能分级分为A组(NYHAⅡ级)、B组(NYHAⅢ级)和C组(NYHAⅣ级),检测血尿酸(UA)、左室射血分数(LVEF)、左室舒张末期内径(LVEDD),比较各组患者血清尿酸水平以及与LVEF、LVEDD的相关性。并根据有无高血压病分为高血压病组和血压正常组,比较两组患者血清尿酸水平。结果:A、B、C组患者血清尿酸水平分别为(363.82±102.36)、(433.23±112.02)、(568.78±170.78)μmol/L,各组差异均有统计学意义(P<0.05);血尿酸水平与LVEF呈负相关(r=-0.326,P<0.05),与LVEDD呈正相关(r=0.351,P<0.05);高血压病组与血压正常组患者尿酸水平分别为(438.81±129.28)、(381.33±143.25)μmol/L,差异有统计学意义(P<0.05)。结论:血清尿酸水平随着心功能分级的增加而升高,可应用于临床评价心功能。高血压病组尿酸水平高于血压正常组,提示血尿酸升高是高血压病发病的预测因子。Objective： To explore the relationship between serum uric acid（UA） levels and cardiac function and blood pressure in patients with chronic congestive heart failure（CHF）.Mothods： A total of 192 patients with CHF whose cardiac function were class Ⅱ-Ⅳ NYHA were selected.Patients were divided into three groups： group A（NYHA Ⅱ）,group B（NYHA Ⅲ） and group C（NYHA Ⅳ）.UA,left ventricular ejection fraction（LVEF）,left ventricular end-diastolic diameter（LVEDD） were determined.The correlation between serum uric acid levels and LVEF,LVEDD of patients in different groups were analyzed.And 192 CHF patients were divided into two groups： normal group（64）and hypertension group（128）according to blood pressure.Serum UA levels were compared in two groups.Results： The serum UA was（363.82±102.36） mmol/L,（433.23±112.02） mmol/L and（568.78±170.78） mmol/L in A,B and C group,respectively.There was significant difference in three groups（P〈0.05）.There was negative correlation between UA level and LVEF.There was positive correlation between UA level and LVEDD.The UA levels was（438.81±129.28） mmol/L and（381.335±143.25） mmol/L in hypertension group and normal group,respectively.Conclusion： Serum UA levels increase with incresing cardiac function and it can be widely used in clinical evaluation of heart function.Higher UA levels in patients with hypertension group suggest that increased serum UA is a predictor of hypertension incidence.
