摘  要:目的全面了解泛耐药鲍氏不动杆菌β-内酰胺类耐药机制。方法收集2008年1-12月住院患者痰液标本中分离的泛耐药鲍氏不动杆菌20株,用聚合酶链反应(PCR)法,分析33种β-内酰胺酶基因和膜孔蛋白carO基因以及外排泵adeB基因。结果 20株泛耐药鲍氏不动杆菌共检出A类β-内酰胺酶基因TEM-1、C类β-内酰胺酶基因ADC-30、D类β-内酰胺酶基因OXA-23,三者阳性率均为100.0%;B类β-内酰胺酶基因未检出;膜孔蛋白carO基因突变率为100.0%,外排泵adeB基因阳性率为100.0%。结论泛耐药鲍氏不动杆菌株对β-内酰胺类药物耐药与产TEM-1、ADC-30、OXA-23β-内酰胺酶,膜孔蛋白carO基因突变和获得adeABC外排泵3种耐药的机制相关。OBJECTIVE To have an all-round view of resistant mechanism of pandrug-resistant Acinetobacter baumannii to β-lactamase antibiotics. METHODS From Jan 2008 to Dec 2008,20 strains of pandrug-resistant A.baumannii isolated from inpatients were collected from a Hospital.Then,33 kinds of beta-lactamase genes,outer membrane porin gene carO,and drug efflux gene adeB were analyzed by PCR. RESULTS In 20 strains of pandrug-resistant A.baumannii,3 kinds of beta-lactam-resistance genes were detected:out TEM-1(class A,100.0%),ADC-30(class C,100.0%),OXA-23(class D,100.0%),but Class B beta-lactamase gene couldn't be detected.All outer membrane porin gene carO had mutations(100.0%),and drug efflux gene adeB were all detected(100.0%). CONCLUSION In this group of pandrug-resistant A.baumannii,producing 3 kinds of beta-lactamase genes:TEM-1,ADC-30,OXA-23,mutation of outer membrane porin carO gene and acquiried drug efflux adeABC play a key role in resistance toβ-lactamase.
