摘  要:目的探讨人乳头状瘤病毒(HPV)分型检测在宫颈癌防治中的应用价值。方法应用流式荧光杂交法(xMAP)技术对1870例可疑宫颈病变妇女行HPV分型检测,其中473例同期行病理组织学诊断,以病理学诊断为金标准分组,正常组81例、宫颈炎组162例、宫颈上皮肉瘤变(CIN)Ⅰ组93例、CINⅡ组65例、CINⅢ组42例、宫颈癌组30例。结果 1870份标本中HPV阳性511份占27.3%,23种HPV亚型均被检测出,前5位依次为HPV16、58、52、33和53;宫颈癌组依次为HPV16、18、58、52和33;HPV检出率随着宫颈病变加重逐渐升高,各组检出率比较差异均有统计学意义(均P<0.05);xMAP法检测宫颈癌的敏感度为93.3%,特异度为69.4%,阳性预测值为16.6%,阴性预测值为99.3%。结论 HPV分型检测有助于早期发现宫颈癌及癌前病变,对宫颈癌筛查及防治具有重要价值;xMAP技术敏感度高,操作方便,价格低廉,适于临床筛查及大范围的流行学调查。OBJECTIVE To investigate the role of HPV genotyping in prevention of cervical cancer. METHODS A total of 1870 samples from women with suspicious cervical lesions were detected to identify HPV genotype by xMAP technology,and 473 cases were performed for pathological diagnosis simultaneously,and the pathologic results were regarded as the gold standard for comparison,81 cases in normal group,162 cases in cervicitis group,93 cases in CINI group,65 patients in CIN Ⅱ group,42 cases of CIN Ⅲ group,30 cases of cervical cancer. RESULTS Totally 511 cases were positive among 1870 cases(27.3%),23 HPV subtypes were detected,the HPV genotype 16,58,52,33 and 53 were detected more frequently.The HPV genotype 16,18,58,52 and 33 were detected more frequently in cervical cancer group.There was a positive correlation between HPV detection rate and cervical lesions,and there was a significant difference(P〈0.05) in the detection rate among various groups.xMAP assay for sensitivity in detecting HPV on cervical cancer was 93.33% and the specificity was 69.4%,positive predictive value was 16.6%,negative predictive value was 99.3%. CONCLUSION HPV genotyping can be helpful for detection;screen and prevent cervical cancer and precancerous lesions early;xMAP technology is sensitively,easily and inexpensively for detecting HPV,and is suitable for clinical screening and large-scale epidemiological investigation.
