经桡动脉途径肾动脉狭窄介入治疗的初步经验

摘　　要：目的探讨经桡动脉途径行肾动脉狭窄介入治疗的安全性和有效性。方法自2010年10月至2011年2月,入选6例经桡动脉途径行肾动脉介入治疗的患者,桡动脉穿刺使用6Fr桡动脉鞘系统(Terum o,Japan),止血使用桡动脉止血带(Terumo,Japan),选择性肾动脉造影使用5FrMPA120cm造影管(COOK,USA),肾动脉支架术使用PCI(percutaneous coronary intervention,PCI)导丝、6FrMPA1 125cm指引导管(Cordis,USA)及肾动脉支架系统。结果肾动脉造影示:4例患者单肾动脉狭窄,2例患者双肾动脉狭窄。6例患者均经桡动脉途径行肾动脉介入治疗成功,无中转股动脉途径,技术成功率达100%。穿刺口恢复良好,无穿刺部位血肿、假性动脉瘤等发生。介入治疗时间(35±4.6)m in,造影剂(典必乐)用量(82.5±7.7)m l,止血时间(4.1±1.1)m in。结论经桡动脉途径行肾动脉狭窄介入治疗技术可行、创伤小、恢复快、并发症少,可能成为肾动脉狭窄介入治疗的另一途径。Objective To explore the feasibility,safety and effectiveness of transradial interventional treatment of renal artery stenosis Methods From October 2010 to February 2011,six patients who underwent transradial renal artery stenting were studied.Radial sheath system（Terumo,Japan） were used to get acess radial artery and Radial tourniquet（Terumo,Japan） was used to stop bleeding.5Fr MPA（COOK,USA） was used to perform selective renal arteriography.Percutaneous renal artery stent system were used to perform renal artery stenting.Results Renal artery angiography showed 4 patients with unilateral renal artery stenosis and 2 patients with bilateral renal artery stenosis.Six patients successfully underwent Transradial Renal artery Angiography（TRA）.Technical success rate was 100%.There were no puncture site hematoma and pseudoaneurysm etc.Procedure time was（35±4.6） minutes.The amount of contrast agent used was（82.5±7.7）ml.Postprocedure bleeding time was（4.1±1.1） minutes.Conclusions Transradial interventional treatment of renal artery stenosis is technology reliable with less invasive,rapid recovery,few complications,and may become an alternative intervention approach for the treatment of renal artery stenosis.
