新健康教育模式在留置鼻胃管老年病人中的应用

摘　　要：目的:探索新健康教育模式在改善老年住院病人安置鼻胃管的耐受性及避免意外拔管中的临床价值。方法:选择2007年6月至2010年5月在我科住院需安置鼻胃管的老年病人82例作为研究对象,按安置鼻胃管时间顺序编号,并随机分组,其中单号为干预组41例,双号为对照组41例。干预组采用新健康教育模式,对照组则采用传统健康教育模式,并对2组对象在安置鼻胃管过程中的耐受性以及在安置成功后意外拔管的发生率进行比较分析。结果:干预组老年病人在置鼻胃管过程中主动配合、置管顺利,置管成功后发生意外拔管2例;对照组中15例病人安置过程口腔盘卷鼻胃管,6例呛咳明显,共21例需要重新安置鼻胃管,置管成功后出现意外拔管12例。干预组在置管时主动配合、置管成功率以及避免意外拔管发生方面明显优于对照组(P<0.01)。结论:新健康教育模式在改善老年病人安置鼻胃管的耐受性和避免意外拔管发生方面有非常明显的作用。Objective：To explore the clinical value of a new health education method in improving aged patients＇ tolerance for nasogastric（NG） tube and in avoiding accidental extubation.Methods：82 aged patients requiring NG tube from June 2007 to May 2010 were enrolled in this study.Patients were randomly divided into two groups following their intubation date：41 odd number cases as the test group and 41 even number cases as the control group.The test group was educated with a new health education method;the control group was educated in the routine way.The two groups were compared and analyzed concerning the patients＇ tolerance during the process of intubation and the incidence of accidental extubation after successful NG intubation.Results：In the test group,patients cooperated actively;their process of intubation was smooth.only 2 cases accidentally removed the tube by themselves.In the control group,21 cases re-indwelled.The reason for re-indwelling was that tube coiled in the mouth in 15 cases,and 6 cases coughed during the process.12 cases accidentally removed the tube by themselves after the successful intubation.The results of the test group were of distinct advantage over the control group in the active cooperation during intubation,the successful rate of intubation,and avoiding accidental extubation（P〈0.01）.Conclusion：The results indicate that the new health education method is of remarkable effect on improving aged patients＇ tolerance during the process of intubation and avoiding accidental extubation.
