寻常型银屑病外周血中Th17细胞及相关因子的表达

摘　　要：目的:检测寻常型银屑病患者外周血中Th17细胞及相关因子IL-17I、L-22表达,分析其与疾病严重程度及病程的相关性,进一步阐明银屑病的发病机制。方法:实验组为寻常型银屑病患者60例,对照组为健康体检者40例,抽取外周血,用三色法流式细胞术检测外周血Th17细胞,ELISA法检测血清中Th17相关因子IL-17I、L-22表达,分析Th17细胞I、L-17I、L-22表达与PASI评分及病程的相关性。结果:实验组外周血Th17细胞百分率(4.71%±2.55%)高于对照组(0.55%±0.39%),差异有统计学意义(P<0.01);Th17细胞与患者PASI评分呈正相关(r=0.47,P<0.01),但与病程无相关性(r=0.09,P>0.05)。实验组血清中Th17相关因子IL-17I、L-22表达(24.02±12.31,18.32±8.14)均高于对照组(7.16±4.04,6.53±4.15),差异有统计学意义(P均<0.01);IL-17I、L-22表达与患者PA-SI评分呈正相关(r=0.46,P<0.01;r=0.51,P<0.01),但与病程无相关性(r=0.03,P>0.05;r=0.19,P>0.05)。结论:Th17细胞可能在寻常型银屑病发病机制中发挥重要作用,可作为判断疾病严重程度的免疫学指标,阻断其相关因子IL-17I、L-22表达可能成为治疗银屑病的新靶点。Objective：To investigate the correlation between psoriasis vulgaris disease severity and disease course with levels of circulating Th17 cells,and related cytokines IL-17,IL-22 in the peripheral blood of patients with psoriasis vulgaris. Methods：Measurements of Th17 cells,IL-17 and IL-22 levels were obtained from blood samples of 60 patients with psoriasis vulgaris and 40 controls.Th17 cells and IL-17,IL-22 levels were determined by triple-color flow cytometry and ELISA respectively.Levels of Th17 cells,IL-17,and IL-22 were then correlated with PSAI scores and psoriasis disease course. Results：The percentage of Th17 cells in peripheral blood of psoriatics was higher than that of normal controls（4.71%±2.55% vs 0.55%±0.39%,P0.01）,and the percentage of Th17 cells was positively correlated with PASI scores（r=0.47,P0.01）.The percentage of Th17 cells did not correlate with disease course（r=0.09,P0.05）.Serum level of IL-17 and IL-22 in psoriatics were higher than those of normal controls（IL-17： 24.02±12.31 vs 7.16±4.04,P0.01;IL-22： 18.32±8.14 vs 6.52±4.15,P0.01）.There was a positive correlation between serum level of IL-17,IL-22 and PASI scores（r=0.46,P0.01;r=0.51,P0.01）,while no correlation was found between serum level of IL-17,IL-22 and disease course（r=0.03,P0.05;r=0.19,P0.05）. Conclusion：Th17 cells maybe play an important role in the pathogenesis of psoriasis vulgaris,and expression level of Th17 cells may be regarded as an immunological indicator for evaluation of disease severity and prognosis.Blocking expression of Th17 cells-related cytokines IL-17 and IL-22 may become a novel target for the treatment of psoriasis.
