维持性血液透析患者前臂瘘与上臂瘘对心功能影响的观察

摘　　要：目的:观察血液透析患者前臂瘘与上臂瘘的血液动力学及对左心功能的影响。方法:通过对47例维持性血液透析患者行心脏及动静脉内瘘彩超,比较分析两种内瘘患者的心输出量(cardiac output,CO)、射血分数(ejection fraction,EF)、内瘘血流量(arteriovenous fistula blood flow,AVFB),动静脉内瘘直径(arteriovenous fistula diameter,AVFD),来评估两组患者心功能,并测定两组患者Kt/V值,评估透析充分性。结果:透析10年以上的上臂瘘患者AVFB、AVFD明显大于前臂瘘患者(P<0.05),上臂瘘患者CO值明显大于前臂瘘患者而EF值明显低于前臂瘘患者(P<0.05),两组患者透析充分性差异无统计学意义(P>0.05)。结论:透析10年以上的患者,透析充分性相当,但上臂瘘对心功能的影响大于前臂瘘。Objective：To observe the influence of forearm fistula and upper arm fistula on cardiac function with maintenance hemodialysis.Methods：Through 47 maintenance hemodialysis patients＇ Colored Doppler supersonic inspects of cardiac and fistula,to comparative analysis two kind fistula patients＇ Cardiac Output（CO）,Ejection fraction（EF）,（AVFB）,Arteriovenous fistula diameter（AVFD）,to evaluate the cardiac function.And test the Kt/v of the two group patients,to evaluate the dialysis adequacy of the two group patients.Results：In patients who dialyse more than 10 years,AVFB and AVFD of upper fistula are higher than forearm fistula obviously（P〈0.05）,CO is higher and EF is lower（P〈0.05）;And there is no significant statistical difference in two groups.Conclusion：In patients who dialyse more than 10 years,the dialysis adequacy of the two groups is matched,but the impact of upper arm fistula on cardiac function is greater than forearm.
