摘 要:目的建立低渗血红蛋白定时释放试验,比较研究慢性肾衰竭(CRF)患者红细胞中血红蛋白的存在状态,探讨其机制和临床意义。方法用低渗HRT五管法,比较研究CRF患者与正常人的血红蛋白释放情况。再用低渗HRT一管法,比较研究5例CRF与4例非CRF肾病患者的血红蛋白释放结果。用交叉电泳研究CRF患者血浆成分对非CRF肾病患者红细胞的影响。结果与正常对照比较,CRF患者标本出现以下特点:白蛋白前移、血红蛋白A2稍前移、定时释放增多、原点残留和泄漏明显。5例CRF的血红蛋白释放强于4例非CRF肾病患者。交叉电泳结果表明,CRF患者血浆成分对非CRF肾病患者红细胞有一定影响。结论与正常人或非CRF肾病患者比较,CRF患者血液成分的电泳释放行为变化较大。白蛋白前移可能是它与血中毒素结合有关、血红蛋白释放方面的变化也可能来自毒素对红细胞的影响。Objective To establish hypotonic hemoglobin release test(HRT)for comparative study of existing status of hemoglobin in red blood cells from normal controls and patients with chronic renal failure(CRF),and exploreed its mechanism and clinical significance.Methods Hypotonic HRT with five tubes method was used for comparative study on results of hemoglobin release between patients with CRF and normal controls.Hypotonic HRT with one tube method was used for comparative study in hemoglobin release results between five CRF patients and four non-CRFs.Cross electrophoresis was used to study the interaction between plasma in CRF patients and red blood cells in non-CRFs.Results In comparison with normal controls,the samples of CRF patients showed following features:antelocation of albumin,slight antelocation of HbA2,increased timed-release,obvious original remains and leakage.The HRT results of five CRF patients were stronger than those patients with non-CRF.The result of cross electrophoresis showed that certain influences were discovered between plasma in CRF patients and red blood cells in non-CRFs.Conclusion The blood electrophoretic release result to CRF patients is different from that of normal controls or non-CRFs.Antelocation of albumin may be due to its binding with blood toxin.Variation to hemoglobin release results perhaps due to the effect of toxin on red blood cell membranes.
