摘  要:目的探讨经皮加压钢板(PCCP)与动力髋螺钉(DHS)对股骨转子间骨折的治疗作用及生物力学性能优劣。方法健康成人股骨成对防腐尸骨湿标本5对,均于股骨大小转子间线形锯断,制成典型的股骨转子间骨折内固定模型。其中,每对左右随机DHS加用空心钉防旋转固定(DHS组),对侧PCCP固定(PCCP组)。模拟髋关节动载荷,对两种治疗方式在髋载荷下的压缩试验及抗扭转性能进行测评。结果 PCCP组及DHS组对髋载荷的压缩反应,张力侧均表现为张应变和张开位移;但PCCP组的对抗断端分离移位能力大大优于DHS组。在压力侧,DHS组的断端间相应靠近嵌插反应较PCCP组大。对骨折而言,这种反应有使压力侧骨折块挤压移位的可能。抗扭转性能DHS组显然不如PCCP组强大。结论 PCCP用于稳定型股骨转子间骨折的固定,其力学性能可保证骨折端的复位和固定要求,固定效果明显优于DHS。Objective To investigate the biomechanical cheracteristics of percutaneous compression plate(PCCP) and the dynamic hip screw(DHS) in the fixation of hip fracture.MethodsFive pairs of male femurs were harvested from fresh adult cadavers.The unsteady hip fracture models were constructed by sawing from the bottom of the femoral neck according to different fixation methods of PCCP and DHS.The anti-pressure and anti-torsion performance were evaluated.ResultsIn the compression responses of the PCCP and the DHS to the hip-loading,the positive strain and displacement were shown on the tension-side,but PCCP provided much better effects in tests of anti-displacement than DHS.On the stress-side,DHS made the fracture-ends closer each other than PCCP,which meant that the fracture was easily displaced by over compression.In the tests of anti-torsion,the PCCP performed better than DHS.Conclusion Compared to DHS,PCCP is better in reduction and fixation of the hip fracture of stable type.
