银屑病性关节炎的超声学特征分析

摘　　要：目的:分析银屑病性关节炎(psoriatic arthritis,PsA)患者的声像图特征。方法:采用高频超声技术分别观察50例确诊PsA、30例寻常型银屑病疑似PsA和30名健康对照者的特定关节部位,并对3组的声像图表现进行比较。结果:50例确诊PsA中有50例发生肌腱或韧带末端炎(占100%);48例发生滑囊炎(占96%)。此组多数有不同程度的骨质侵蚀(占82%)。30例疑似PsA的患者中,有30例发生肌腱末端炎(占100%);18例发生滑膜炎(占60%),均无骨质侵蚀的改变。30例健康人高频超声无任何阳性表现。结论:肌腱或韧带末端炎和滑囊炎应作为PsA的诊断标准之一,疑似PsA的患者应作为PsA的可疑病例长期随访。Objective： To analyze the ultrasonographic characteristics of joints in patients with psoriatic arthritis （PsA）. Methods： The joints of 50 cases of psoriatic arthritis, 30 cases of suspected psoriatic arthritis and 30 healthy controls were examined by high frequency ultrasonograpy. Results： The inflammation of tendon and ligament was found in all 50 PsA cases （100%） and all the 30 cases of suspected PsA, while 48 of 50 PsA cases and 18 out of 30 suspected PsA cases were found having synovitis （96 % ）. Bone absorption or osteolysis was also demonstrated by uhrasound in 82% of PsA patients （48/50）. But in suspected group, none of the cases had osteolysis showed. No pathological findings were demonstrated in 30 healthy controls. Conclusion： Uhrasonography is a reliable method for the diagnosis of PsA. Inflammation of tendons and ligament, synovitis, and bone absorption or osteolysis are the most con-anon findings under ultrasound in PsA patients.
