摘 要:目的本实验研究高渗盐水对大鼠肝脏缺血再灌注损伤的影响及可能机制。方法健康雄性SD大鼠96只,随机分为假手术组(Sham组)、缺血再灌注组(IR组)、高渗盐水组(HTS组)、丹参组(SM组),每组又按再灌注时间(1、3、6、12 h)分为4个亚相,每组各亚相为6只。复制70%肝缺血再灌注模型(Pringle's法),测血清ALT、AST水平及肝脏组织MDA水平;常规病理及电镜下观察大鼠肝脏病理学变化;用免疫组化(streptavidin-perosidase,SP)法检测Bcl-2及Bax的表达。结果 IR组、SM组及HTS组各时点AST、ALT、MDA及Bax水平较Sham组高,且Bcl-2表达水平较Sham组低,肝组织病理学和超微结构损害明显程度高于Sham组,差异有统计学意义(P<0.01);HTS组及SM组各时点AST、ALT、MDA及Bax水平较同时点IR组低,且Bcl-2水平较IR组高,肝组织病理学和超微结构损害明显轻于IR组,差异有统计学意义(P<0.01);HTS组各时相AST、ALT、MDA及Bax水平较同时相SM组低,且Bcl-2水平较SM组高,肝组织病理学和超微结构损害明显轻于SM组,差异有统计学意义(P<0.01)。结论 SM与HTS对肝脏缺血再灌注损伤有保护作用,且后者效果更优,HTS可以通过抑制Bax的表达,促进Bcl-2的表达水平,抑制肝细胞凋亡,对肝脏缺血再灌注损伤有保护作用。Objective This experiment studies the (HIRI) in rats and the feasible mechanisms. Methods effect of Hypertonic Saline on liver ischemia reperfusion injury Ninety - six healthy male SD rats were randomly divided into four groups: sham operation group (Sham group), ischemia reperfusion group (IR group), HTS group and salvia mihiorrhiz group (SM group). Levels were divided at the four time point 1, 3, 6 and 12 hours of reperfusion, six in each. HIRI rat models were established by Pringle's method to determine the levels of serum AST (alanine - aminotransferase), ALT ( alnine - aminotransferase), MDA ( malondial dehyde) to observe the changes of liver histopathology and ultra - struc- ture. SP method was used to inspect Bax and Bcl -2's expression. Results At 1, 3, 6, 12h after reperfusion, the ser- um levels of ALT, AST, MDA and Bax were significantly higher in HTS group, SM group and IR group than those in Sham group, levels of ALT, AST, MDA and Bax in group HTS and SM were lower than those in group IR. Group HTS was the lowest (P 〈 0. 01 ), but Bcl - 2 were significantly lower in HTS group, SM group and IR group than those in Sham group, levels of Bcl -2 in group HTS and SM were higher than those in group IR. Group HTS was the highest (P 〈0. 01 ). The pathological and ultra - structure changes of liver were significantly slighter in HTS group and SM group than those in IR group. The HTS group was the slightest. Conclusion SM showed less preventive effect on hepatic ischemia reperfusion than Hypertonic saline. HTS showed preventive effect by increasing the level of Bcl - 2 and decreasing the level of Bax and hepatocellular apoptosis. Hypertonic saline had a protective effect on liver ischemia reperfusion injury.
