吡格列酮对2型糖尿病患者血压超敏C反应蛋白和肾功能的影响

摘　　要：目的观察吡格列酮对2型糖尿病患者血压、超敏C反应蛋白和肾功能的影响。方法 72例24h尿白蛋白排泄量(UAE)在30～300mg的2型糖尿病患者在常规降糖治疗的同时随机分为治疗组(吡格列酮30mg/d)36例和对照组36例,疗程24周,分别观察治疗前后2组空腹血糖(FBS)、餐后2h血糖(2h PBG)、糖化血红蛋白(HbA1c)、血压(BP)、血尿素氮(BUN)、血肌酐(Cr)、超敏C反应蛋白(us-CRP)和24h UAE的变化,并计算内生肌酐清除率(Scr)。结果治疗组治疗前后FBS、2h PBG、HbA1c、BP、BUN、us-CRP和24hUAE显著下降(P<0.01),Scr显著上升(P<0.01);对照组FBS、2h PBG、HbA1c明显下降(P<0.05),BP、BUN、Scr、us-CRP24h和UAE无差异性(P>0.05);2组间治疗后比较差异具有显著性(P<0.01)。结论吡格列酮不但能够降低2型糖尿病患者的血压、超敏C反应蛋白,而且对患者的肾功能具有保护作用。Objective To observe the effect of pioglitazone hydrochloride on the bood pressure,us-CRP and the renal function in type 2 diabets.Methods To tally 72 patients were randomly and double-blindly divided into therapy group（n=36,pioglitazone hydrochloride 30mg,qd） and control group（n=36）,for 24 weeks.FBS,2h PBG,HbA1c,BP,BUN,Scr,us-CRP and 24h UAE at the beginning and after 24 weeks of treatment were monitored respectively.Results The levels of FBS,2h PBG,HbA1c,BP,us-CRP and 24h UAE of therapy group decreased significantly（P0.01）,the levels of Scr increased significantly（P0.01）.The levels of FBS、2h PBG and HbA1c of control group decreased significantly（P0.05）,the levels of BP,BUN,Scr,us-CRP and 24h UAE had no significant difference（P0.05）.There was significant difference between 2 groups（P0.01）.Conclusions Not only pioglitazone hydrochloride can decrease the levels of FBS,2h PBG,HbA1c,BP,us-CRP and 24h UAE in patents with type 2 diabets,but also protect renal function.
