介入栓塞治疗剖宫产后晚期产后出血12例临床分析

摘　　要：目的:进一步探讨髂内动脉介入栓塞治疗剖宫产术后晚期大量反复产后出血的应用价值。方法:对12例剖宫产术后晚期大量反复产后出血患者,采用seldinger技术行双侧髂内动脉插管和造影,了解出血部位及出血动脉分支,用明胶海绵颗粒栓塞双侧髂内动脉前干。结果:12例剖宫产术后晚期反复活动性出血患者均在25～50 min内完成栓塞治疗。12例立即止血,血压回升,生命体征平稳。术后12例患者痊愈出院。结论:髂内动脉介入栓塞治疗剖宫产术后晚期大量反复产后出血,止血迅速、准确,不良反应小,患者康复快,能保留子宫,维持患者正常生育及内分泌功能,值得推广应用。Objective To investigate the effect of the intervention embolism in the treatment of patients of late postpartum haemorrhage after cesarean section in the further step.Method We made bilateral internal iliac intubation and angiography by selginger skill successfully to define the bleeding site and the arterial branches with hemorrhage.We used gelfoam particles to clog the anterior branches of bilateral internal iliac arteries.Results The 12 patients of late postpartum haemorrhaged repetitiously after cesarean section were cured by intervention embolism during 25~50 min.They were stopped bleeding immediately with their blood pressure rebounded later.And their vital signs were stable.All the patients were healed and discharged from hospital.Conclusion Intervention embolism,which is used to cure the patients of late postpartum hemorrhage after cesarean section,should be worth having an extensive use.It helps doctors to stop bleeding rapidly and accurately,and has few side effects.This method retains their uterus so that the patients can maintain their normal function of the fertility and internal secretion.
