摘 要:目的:观察手足口病(HFMD)患儿早期外周血中T淋巴细胞亚群(CD3+T、CD4+T、CD8+T)、B淋巴细胞(B)、自然杀伤细胞(NK)、以及免疫球蛋白和补体水平的变化,探讨其早期免疫功能改变。方法:收集杭州地区2010年4月~2010年6月手足口病患儿135例作为研究对象,48例正常的健康儿童作为对照组;分别应用流式细胞术(FCM)检测CD3+T、CD4+T、CD8+T、B以及NK细胞绝对数值,计算CD4/CD8比值;用免疫透射比浊法测定IgA、IgG、IgM、C3及C4。结果:与对照组比较,患儿的CD3+T、CD4+T、CD8+T、B、NK细胞的绝对数值显著升高,差异有统计学意义(P<0.05),血清IgM、IgG显著增高,差异有统计学意义(P<0.05),IgA显著降低,差异有统计学意义(P<0.05);血清C3及C4水平无明显变化,差异有统计学意义(P>0.05)。结论:手足口病患儿早期出现机体免疫功能紊乱,早期检测免疫球蛋白及CD3+T、CD4+T、CD8+T、B、NK细胞数的变化对于判断手足口病患儿病情及预后具有重要作用。Objective To investigate the early lever of T-cell subpopulations(CD3+T,CD4+T,CD8+T),NK,B,immunoglobulin and complement in child patients with hand-foot-mouth disease.Method Total 135 child patients with hand-foot-mouth disease in Hangzhou from April 2010 to June 2010 and 48 control normal subjects were included in this study.CD3+T,CD4+T,CD8+T,B and NK were detected by flow cytometry(FCM) and calculated the ratio of CD4 /CD8,immunoglobulin(IgA,IgG,IgM) and complement(C3,C4) were analyzed by immune turbidimetry assay.Results Compared with the control normal subjects,the levels of CD3+T,CD4+T,CD8+T,B and NK were increased,and the ratio of CD4 to CD8 were reduced(P0.05) in patients with hand-foot-mouth disease.The level of IgM and IgG were higher than those in control normal subjects,but the level of IgA was lower(P0.05).The level of C3 and C4 in the two groups were not obviously different(P0.05).Conclusion The early immune function is disorder in child patients with hand-foot-mouth disease;early detection of CD3+T,CD4+T、CD8+T,B and NK cells changes in child patients with HFMD to determine the condition and prognosis play an important role.
