摘  要:目的分析广州市0~14岁儿童卡介苗特异性Th1细胞免疫水平。方法随机抽取288例广州市妇女儿童医疗中心儿童院区健康体检的0~14岁儿童静脉抗凝血,经BCG刺激后,用酶联免疫吸附试验(ELISA)检测Th1细胞因子(IFN-γ,TNF-α和IL-2)的水平,并用流式细胞术(FACS)检测IFN-γ和TNF-α的细胞来源。结果 BCG刺激可显著诱导儿童外周血细胞产生IFN-γ(median=726.37 pg/ml),TNF-α(median=934.98 pg/ml)和IL-2(median=48.25 pg/ml)。与不刺激组相比,差别具有统计学意义(n=288,P<0.001)。此外,IFN-γ的产生水平与年龄呈一定的正相关(r2=0.4069,P<0.05)。流式分析结果表明,BCG刺激可诱导CD4+T细胞和非CD4CD8细胞分泌IFN-γ或TNF-α,而不诱导CD8+T细胞产生细胞因子。经BCG刺激后,分别有35.71%和39.29%的儿童CD4+T细胞能够产生IFN-γ和TNF-α。结论广州市0~14岁儿童外周血细胞经BCG刺激后,主要是由CD4+T细胞和非CD4CD8细胞产生Th1细胞因子,但是CD4+T细胞能够产生Th1细胞因子的儿童比例较低,这可能是卡介苗接种免疫效果并不理想的原因之一。This study is aimed to investigate BCG-specific Th1 cellular immune responses in children aged from 0 to14 years old in Guangzhou.We randomly recruited 288 cases of healthy check-up children from the Children's Hospital of Guangzhou.Th1 cytokines(IFN-γ,TNF-α,and IL-2) were detected by ELISA following stimulation with BCG.The production of IFN-γ and TNF-α following BCG stimulation were further analyzed by FACS.Significantly higher levels of IFN-γ(median = 726.37 pg/ml),TNF-α(median = 934.98 pg/ml) and IL-2(median = 48.25 pg/ml) were detected following BCG stimulation as compared with the unstimulated condition(n = 288,P 0.001).And IFN-γ level was positively related with the age of the children(r2 = 0.4069,P 0.05).In addition,IFN-γ and TNF-α were mainly produced by CD4+ T cells but not CD8+ T cells,after BCG stimulation.IFN-γ was also produced by nonCD4CD8 cells.In conclusion,only 35.71% to 39.29% of children are demonstrated to produce IFN-γ and TNF-α by CD4+ T cells following stimulation with BCG,which might be the possible reason for the limited protective effect of BCG vaccination.
