超声弹性成像指导泼尼松治疗亚急性甲状腺炎的初步应用

摘　　要：目的探讨超声弹性成像指导泼尼松治疗亚急性甲状腺炎(subacute thyroiditis,SAT)的临床意义。方法对临床疑似SAT的46例患者行超声引导下细针穿刺活检,将确诊为SAT的39例患者随机分成两组行泼尼松治疗,治疗组20例(30个病灶)用超声弹性成像观察疗效并指导用药,对照组19例(28个病灶)根据临床症状指导用药,并在停药后跟踪观察12个月。结果 SAT患者患侧甲状腺呈轻中度肿大,部分呈双侧发病,两组病例治疗过程中均未见明显不良反应,治疗组治愈率96.77%,对照组治愈率71.43%,两组比较差异有统计学意义(P<0.05);而两组泼尼松的疗程、总用药量差异无统计学意义(P>0.05)。结论 SAT超声弹性图像具有一定的特征性表现,超声弹性成像可指导泼尼松治疗SAT。Objective To explore the clinical effect of treating subacute thyroiditis with prednisone under the guidance of ultrasound elastography. Methods Forty-six patients suspected of subacute thyroiditis（SAT） received fine needle aspiration biopsy（FNAB） under the guidance of ultrasound elastography,and thirty-nine patients diagnosed with SAT were randomly divided into two groups： the control group received prednisone according to clinical symptoms,and the tested group received prednisone under the guidance of ultrasound elastography.Patients of both groups were follow-up for twelve months after drug discontinuation. Results Patients with SAT had mild to moderate thyroid enlargement in affected side,some patients had bilateral thyroid enlargement.Patients in both groups showed no side effect in the process of treatment.The curative rate of tested group and control groups were 96.77% and 71.43%,respectively,which showed significant difference between two groups（P0.05）.There were no significant differences on dosage and course of prednisolone between two groups（P0.05）. Conclusion The sonographic image of patient with SAT has a characterized manifestation.Using ultrasound elastography to observe the efficacy of prednisone for patients with SAT has an important clinical implication on guidance for drug usage.
