阻塞性睡眠呼吸暂停低通气综合征患者代谢紊乱的研究

摘　　要：目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者代谢紊乱特征及OSAHS与代谢综合征(MS)的关系。方法 40例OSAHS患者,重度组20例,轻中度组20例,并选非OSAHS对照组20例,分别测量腰围、体重指数、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)等指标,三组间进行比较,并研究以上指标与OSAHS严重程度之间的关系。结果 OSAHS患者中MS占45%,高于对照组(χ2=5.275,P<0.05)。轻中度组、重度组受试者腰围、收缩压、舒张压均高于对照组(F值分别为27.12、22.3、24.35,P<0.01);轻中度组、重度组受试者FPG、FINS、HOMA-IR高于对照组,重度组以上指标高于轻中度组(F值分别为33.24、143.02、133.53,P<0.01);轻中度组、重度组HDL-C低于对照组,重度组低于轻中度组(F=42.6,P<0.01)。AHI与FPG、FINS、HDL-C、HOMA-IR呈正相关(r值分别为0.47、0.55、0.44、0.63,均P<0.01);LSaO2与FPG、FINS、HOMA-IR呈负相关(r值分别为-0.56、-0.34、-0.47,P<0.01)。结论 OSAHS患者中MS所占比例高于健康人群,OS2AHS越严重,胰岛素抵抗及代谢紊乱越明显。Objective To explore the status of metablic disorder and insulin resistance in patients with obstructive apnea-hypopnea syndrome（OSAHS）.Methods 40 OSAHS patients were enrolled in study,20 were grouped into severe group and 20 into mild-moderate group according to apnea-hypopnea index（AHI）,20 healthy people were selected as control group.Waist circumference,body mass index,triglyceride（TG）,high-density lipoprotein（HDL-C）,fasting blood glucose（FPG）,fasting insulin（FINS）,insulin resistance index（HOMA-IR） were recorded and compared in 3 groups.The relationship between OSAHS and metablic disorder were analyzed.Results The Prevalence of metalbic syndrome in patients with OSAHS was signifcantly higer than that in control group（P0.05）.Waist circumference,blood pressure in servere and mild-moderate group were significantly higher than that in control group（P0.01）;FPG,FINS,HOMA-IR in servere and mild-moderate group were significantly higher than that in control group,HDL-C in servere and mild-moderate group was significantly lower than that in control group,and that in servere group was significantly lower than that mild-moderate group（P0.01）.AHI was significantly positively correlated with FPG,FINS,HDL-C,HOMA-IR;LSaO2 was negatively significantly correlated with FPG,FINS,HDL-C,HOMA-IR（P0.01）.Conclusion The prevalence of MS were higher in OSAHS patients.Metabolic disorder were correlated with severity of OSAHS.
