神经源性膀胱患者排尿情况观察表的设计及临床应用

摘　　要：目的探讨自行设计的神经源性膀胱患者排尿情况观察表的临床应用效果。方法自行设计的排尿情况观察表分为表1和表2两部分,观察表1由患者或其照顾者记录每个时间段膀胱的入量和出量;观察表2则由护士每天记录1次患者各种排尿方式下的排尿时间、次数、量和尿液性质。结果通过3年的临床应用,该评定表能客观、全面、准确地记录患者排尿情况。患者对护理服务的不满意为0;患者及其照顾者配合程度:非常配合47例(74.60%),较配合14(22.23%),不配合2例(3.17%)。结论自行设计的排尿情况观察表,方便家属及照顾者记录和掌握排尿规律,可为医生、护士指导患者实施膀胱功能训练提供依据,并提高了患者对护理的满意度和配合,有利于患者膀胱功能的恢复。Objective To investigate the application effectiveness of self-designed tables for observing urination.Methods The self-designed tables consisted of Table 1 and Table 2.The table 1 was used by patients and their caregivers to record bladder output and input in each time segment.The table 2 was used by nurses to record the time,frequency and volume of voiding and the nature of the urine once per day.Results Through a three years of application,the self-designed tables could objectively,comprehensively and accurately record the voiding condition of patients with neurogenic bladder.All patients were satisfied with the nursing care.Patients and their caregivers were cooperative in 47 cases（74.60%）,moderately cooperative in 14（22.23%）,and uncooperative in 2（3.17%）.Conclusion The self-designed tables are convenient for patients＇ families and caregivers to record and master the regular patterns of urination,and may provide a basis for doctors and nurses to guide the implementation of bladder training program.At the same time,the application of the tables can increase patients＇ satisfaction and cooperation with care and therefore promote the recovery of bladder function.
