徐州市健康体检者白细胞尿流行现况及危险因素分析

摘　　要：目的本研究通过回顾性分析徐州市健康体检者白细胞尿的流行现况及相关危险因素,为本地区白细胞尿的防治提供决策依据。方法资料选自徐州市中心医院体检中心数据库2006-2009年完成的体检有效资料23 741份,根据镜检白细胞尿诊断结果,使用卡方检验、t检验、Logistic回归等方法进行现况描述和相关因素研究。结果徐州市健康体检人群中白细胞尿患者2 802例,白细胞尿的总患病率为11.80%。男性患病率为4.92%,女性患病率为23.58%(χ2=1 846.707,P=0.000)。随年龄组增加男性女性患病率都持续增加。高龄、女性、蛋白尿是白细胞尿独立的危险因素。结论徐州地区健康体检者白细胞尿患病率较高,女性明显高于男性,患病率随年龄增长持续增加。人群预防和控制泌尿系感染已经迫在眉睫,发现白细胞尿阳性要复查和确诊,不可盲目使用抗生素,尤其是是伴有蛋白尿者对保护肾脏功能,及时控制感染都有很高的临床价值。Objective To provide a basis for the prevention and treatment of leukocyturia through retrospectively analyzing prevalence rate and risk factors of leukocyturia among physical examinees in Xuzhou city.Methods Data of 23 741 cases from Xuzhou Center Hospital between 2006 and 2009 was retrospectively analyzed.According to microscopic examination,prevalence rate and risk factors of leukocyturia were investigated using chi-square test,t-test and Logistic regression.Results Among the physical examinees,2 802 had leukocyturia,with a total prevalence rate of 11.80%（male：4.92%,female：23.58%,χ2= 1 846.707,P=0.000）.The prevalence rate for males and females increased with age.The elderly,female and proteinuria were independent risk factors for leukocyturia.Conclusion The prevalence rate of leukocyturia is high and increases with age in Xuzhou city,and the prevalence rate in females is significantly higher than that in males.The prevention and control of urinary system infection is imminent,and rechecking is imperative to diagnose leukocyturia.The avoidance of indiscriminate use of antibiotics is very important for protecting kidney function and controlling infection,especially in leukocyturia associated with proteinuria,
