COPD急性加重期降钙素原与超敏C反应蛋白检测临床意义

摘　　要：目的探讨血清降钙素原对慢性阻塞性肺疾病急性加重期(AECOPD)患者的诊断、疗效观察中的应用价值。方法检测45例AECOPD患者和25例健康对照组血清PCT、His-CRP、WBC的浓度,并动态观察以上各指标在AECOPD患者治疗前后的变化。结果 PCT的敏感性及特异性均高,可作为诊断AECOPD的可靠指标;PCT与His-CRP治疗前后比较有显著差异(P<0.05);WBC治疗前后变化不明显(P>0.05)。结论 PCT是诊断AECOPD的较好指标;His-CRP与PCT在AECOPD患者经有效抗感染治疗后均可发生显著变化,联合检测His-CRP和PCT对AECOPD的疗效观察具有重要的临床意义。Objective To investigate clinical application value of the serum procalcitonin（PCT） and the relationship between PCT and diagnosis,treatment and condition assessment in patients with Acute Exacerbations of Chronic Obstructive Pulmonary Disease（AECOPD）.Methods 45 cases of patients with AECOPD and 25 cases of healthy controls were tested by PCT,high-sensitivity C-reactive protein（his-CRP）,white blood cell（WBC） in 24 hours.The measures were observed dynamically on various inflammatory indicators in the patients before and after the treatment of AECOPD.Results The index of the sensitivity of PCT（87%） and specific degrees（80%） were higher than the index of his-CRP and WBC,which indicated that PCT was a good indicator of diagnosing AECOPD.The measures of PCT and his-CRP before and after treatment were significantly different（P0.05）,while WBC before and after treatment did not change significantly（P0.05）.Conclusions（1） The measurements of the PCT level are helpful for diagnosing AECOPD;（2） The combined detection of his-CRP and PCT on the efficacy of AECOPD has important clinical significance.
