BiPAP呼吸机治疗老年COPD合并Ⅱ型呼吸衰竭的疗效分析

摘　　要：目的探讨双水平气道正压(BiPAP)对老年慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的治疗作用。方法观察17例老年COPD合并Ⅱ型呼吸衰竭患者应用BiPAP呼吸机辅助治疗前后患者血气及临床情况的变化。结果 17例中好转15例(88.0%),患者PaO2,SaO2明显上升,PaCO2明显下降,临床情况改善。1例病情加重改为有创通气,1例病情严重,拒绝气管插管死亡。结论 BiPAP呼吸机辅助通气治疗COPD合并Ⅱ型呼吸衰竭疗效肯定,减少了气管插管,有利于患者康复。Objective To discuss the treatment of elderly patients of chronic obstructive pulmonary disease（COPD） with type Ⅱ respiratory failure by using facial mask bilevel positive airway pressure（BiPAP）.Methods Observations were made on the changes of blood gas and clinical situation in 17 elderly COPD patients with type Ⅱ respiratory failure before and after using BiPAP ventilator-assisted treatment.Results In the 17 patients,15 cases improved（88.0%）： PaO2 and SaO2 increased significantly,PaCO2 decreased significantly,and clinical situation improved.1 case changed to use invasive ventilation,and 1 case died of refusing to accept intubation.Conclusion It is effective for BiPAP ventilator-assisted to treat COPD patients with type Ⅱ respiratory failure,which is not only conducive to patients rehabilitation,but also decreases intubation.
