冠心病慢性心力衰竭常见中医证型与心脏超声结果关系的研究

摘　　要：目的:探讨冠心病慢性心力衰竭患者常见中医辨证分型与心脏超声之间的量化关系。方法:对125例冠心病慢性心力衰竭患者辨证分型,并依据其身高、体重和心脏彩超相关结果,分别计算心脏指数(CI),左室质量指数(LVMI),初步分析冠心病慢性心力衰竭患者中医证型与CI、LVMI之间的相关性。结果:气虚证CI最高,血瘀证次之,阴虚证,阳虚证,水停证逐渐减低;水停证LVMI最高,阳虚证次之,阴虚证、血瘀证、气虚证逐渐减低。气虚证与血瘀证,气虚证与阴虚证两组间CI比较有显著性差异(P<0.01);气虚证、血瘀证、阴虚证三组间LVMI比较均有显著性差异(P<0.01);阳虚证与水停证两组间CI、LVMI比较无明显差异(P>0.05),但与气虚证、血瘀证、阴虚证组间比较均有差异(P<0.01或P<0.05)。结论:冠心病慢性心力衰竭常见中医证型与心脏超声结果有一定的相关性,CI和LVMI可以作为冠心病慢性心力衰竭中医辨证分型的参考指标。Objective： investigate the relation of the common syndrome of chronic heart failure coronary heart disease and the result of cardiac uhrasonography. Methods： analysis the differential aspects of 125 patients with chronic heart failure coronary heart dis- ease, calculate the Cardiac Index （CI）, Left Ventricular Mass Index （LVMI）. Results：Qi deficiency CI highest, followed by blood stasis, yin deficiency, yang deficiency permits, certificates gradually reduce water retention; water retention LVMI highest card, followed by Yang deficiency, Yin deficiency, blood stasis, qi deficiency gradually reduced. Qi deficiency and blood stasis, qi deficiency and yin deficiency CI between the two groups were significantly different （P〈0.01） ; qi deficiency, blood stasis, yin deficiency LVMI among the three groups were significantly different （P〈0.01） ; yang deficiency syndrome and water retention certificates between the two groups CI, LVMI was no significant difference （P〉0.05）, but with qi deficiency, blood stasis, yin deficiency were differences among groups （P〈0.01 -0.05）. Conclusion：there are certainly relationship between the common syndrome of chronic heart failure coronary heart disease and the result of cardiac uhrasonography, and the cardiac index and the left ventricular mass index could be a reference to the TCM-bion zheng with chronic heart failure coronary heart disease.
