手部热压伤序贯治疗策略的应用初探

摘　　要：目的探讨应用序贯策略治疗手部热压伤的效果。方法对我科2009年1月至2010年12月收治的13例手部热压伤患者,根据损伤表现和病情演变进行序贯治疗。结果 1例经局部换药创面转归自愈,12例行清创后损伤范围、程度均有不同程度改善,移植组织全部成活,本组无感染病例。术后随访3-12个月,患者对外形及功能均感满意。结论序贯治疗有利于争取间生态组织,促进热压创面转归,降低损伤判断及手术修复的难度,可最大限度保留并恢复患手的功能和外形。Objective To investigate the therapeutic efficacy for hand heat-press injury under Sequential-Therapy Strategy. Methods From Jan. 2009 to Sep. 2010, thirteen patients with hand heat-press injury were treated by Sequential- Therapy according to the burn situation and their succession. Results One case was healed by dressing exchange, the other 12 showed a significant improvement of both extension and range of the burn after initial debridement. All of the autologous grafts survived without infection after transplantation. Both appearance and function restoration of the hand were satisfactory. All patients were followed up for 3-6 months. Conclusion The application of Sequential-Therapy Strategy in treating hand heat-press injury is beneficial to retain parabiosis tissue, promote the turnover of the raw surface of the heat-pressed wound and decrease the difficuhy in the judging and repairing procedures, which largely restore hand function and its appearance.
