泌尿生殖道支原体属感染男性不育患者精子DNA完整性分析及应用

摘　　要：目的探讨泌尿生殖道支原体属感染男性不育患者精子DNA完整性的改变及其临床意义。方法对79例泌尿生殖道支原体属感染男性不育患者(患者组)和16例健康生育男性(对照组)同时进行精子染色质扩散试验(SCD)、与精液常规参数检测;在患者组随机选取32例进行抗菌药物治疗,用SCD试验检测治疗前后的精子DNA损伤情怳。结果患者组的精子DNA碎片化指数(DFI)为(35.7±14.7)%,对照组精子DFI为(11.2±5.9)%,2组差异有统计学意义(P<0.01);32例患者治疗前的精子DFI为(35.3±14.2)%,抗菌药物治疗后的精子DFI为(19.6±9.7)%,2组差异有统计学意义(P<0.01);患者治疗前后的精子密度、活动率比较,差异无统计学意义;患者治疗后的精子畸形率明显减少,差异有统计学意义(P<0.05)。结论泌尿生殖道支原体属感染男性不育患者的精子DNA损伤,明显高于健康生育男性,相应抗菌药物治疗可以降低感染造成的精子DNA损伤,精子DNA完整性分析在支原体感染不育男性诊疗中,比精液常规参数检测更有价值。OBJECTIVE To discuss the clinical significance and application of sperm DNA integrity of male infertility patients with mycoplasma infections. METHODS A total of 79 cases of infertility patients with mycoplasma infection and 16 healthy fertile men were taken sperm chromatin diffusion test （SCD） and conventional semen parameters detection, and 32 patients were selected randomly from patient group that were treated with antibiotics, the sperm DNA damage were analyzed before and after the treatment with SCD test. RESULTS There was a significant difference between two groups and the DFI were （35.7±14.7）% and （11.2± 5.9）% respectively. In the patients group the DFI was （35.3±14.2）% before treatment. In contrast, the DFI was dawn to （19. 6±9. 7）% after anti-infection treatment. The difference was significant. There were no significant difference in density and activity between before and after the treatment. The abnormal sperm was significantly reduced after treatment, and the difference was statistically significant. CONCLUSION The sperm DNA damage of infertile patients with mycoplasma infections are significantly higher than that of fertile men. Antibiotic treatment can reduce sperm DNA damage caused by mycoplasma infection. Sperm DNA integrity analysis is more valuable than normal semen parameters in the diagnosis and treatment of infertile men with mycoplasma infections.
