集束干预策略对ICU中心静脉导管相关性感染的影响

摘　　要：目的探讨集束干预策略对ICU中心静脉导管相关性感染(CVC-RI)发生的影响。方法对2005年8月-2007年8月使用中心静脉导管214例患者设为对照组,对其进行回顾性调查,了解ICU中心静脉导管相关性感染发生情况;采用集束干预策略对2007年9月-2009年9月使用中心静脉导管305例设为试验组进行管理,比较两组CVC-RI的发生情况。结果对照组CVC-RI发生率为10.46%,平均住院(6.88±0.89)d,ICU总住院费用(32 015±1306.57)元;试验组CVC-RI发生率为6.91%,平均住院(4.76±0.91)d,ICU总住院费用(22 386±1236.48)元。结论采用集束干预策略是预防和减少ICU中心静脉导管相关感染的有效方法,能降低ICU患者CVC-RI的发生率、缩短在ICU的住院天数,减轻患者的经济负担。OBJECTIVE To investigate the effect of cluster interfere strategy on the CVC related （CVC-RI） infection in ICU. METHODS In our ICU we investigated retrospectively 214 patients with CVC from Aug 2005 to Aug 2007 （group A）. The CVC-RI infection rate, were calculated from Sep 2007 to Sep 2009. Cluster interfere strategy was used to manage 305 ICU patients with CVC （group B）. Then we compared the CVC-RI infection rate between these 2 groups. RESULTS Group A CVC-RI infection rate was 10.46G , hospital days was（6. 88±0.89） d,and the cost of hospitalization was （32015±1306. 57） yuan; Group B CVC-RI infection rate was 6. 91%, hospital days was（4.76 ±0.91） d, and the cost of hospitalization was 22386±1236.48 Yuan. CONCLUSION Using the cluster interfere strategy in ICU CVC management can prevent or decrease the CVC-RI infection rate, decrease the hospital days and decrease the economic burden of the patients.
