恩替卡韦治疗36例活动性肝炎肝硬化1年的疗效观察

摘　　要：目的观察恩替卡韦治疗活动性肝炎肝硬化一年的临床疗效。方法将68例HBV—DNA定量呈阳性的活动性肝炎肝硬化分为二组，治疗组38例，对照组32例。两组均用基础治疗，治疗组加用恩替卡韦0．5mg，口服，每日一次，疗程为一年。结果治疗组和对照组的总胆红素、ALT、白蛋白、凝血酶原活动度（PTA）的改变分别为299±127和640±283，54．7±23．4和1304±570，35．7±29和306±35，58．6±115和41．0±13．4。均有显著差异（P〈0.01）。两组的HBV—DNA阴转率分别为9722％（35／36）@12．5％f4／32）。两组的病死率分别为2．78％（1／36）和1875％（6／32）。结论恩替卡韦治疗活动性肝炎肝硬化能明显改善活动性肝炎肝硬化患者的肝功能和预后。Objective To investigate curative effect of treating 36 eases in the active stage of hepatitis with hepatocirrhosis with entecavir for one year. Methods We divided 68 cases of hepatitis with hepatocirrhosis of positive HBV-DNA into 2 groups, of which there were 36 treat groups and 32 contrast groups. Two groups were both treated with foundation , in addition treat groups were given 0.5mg entecavir which were taken one time a day per os for one year. The course of treatment is one year. Results One year later,changes in total bilirubin, ALT, albumin, prothrombase activity degree （PTA） of treat groups and contrast groups were respectively 29.9±12.7 and 64.0±28.3, 54.7±23.4 and 130.4±57.0, 35.7±2.9 and 30.6±3.5, 58.6±11.5 and 41.0±13.4. There was a remarkable difference . HBV-DNA of two groups was 97.22% （35/36） and 12,5 （4/32） respectively. Case-fatality rate of two groups was 2.78% （1/36） and 18.75% （6/30） respectively. Conclusion Result of this study shows that entecavir can obviously improve liver function and prognosis of patients in the active stage of hepatitis with hepatoeirrhosis.
