临床免疫学试验标本稀释液的实验评价及选择原则

摘　　要：目的 对临床免疫学试验18种常用标本稀释液进行实验评价,确定临床免疫学实验室最佳通用稀释液,并提出选择和评价临床免疫学标本稀释液的基本原则.方法 对标本稀释液的空白值、最小检测限、干扰试验、线性评价、重复性等五种参数进行检测,并依据制订的判断标准及评分方法进行综合评分,以综合评分高者为临床免疫学试验最佳通用稀释液.结果 18种标本稀释液中专用稀释液评价得分最高,其次是自配的含4 g/L小牛血清的等渗pH7.4 PBS,混合人血清和不含胶体介质的稀释液得分最低,不同稀释液主要是对检测指标的分析灵敏度、回收率和相关系数等三个参数产生影响.结论 专用稀释液应用范围窄,含4 g/L小牛血清的等渗pH7.4 PBS可以作为临床免疫学实验室高值检测指标的通用稀释液,它具有配制简单、使用方便、应用范围广等特点.Objective To determine the best general diluent by evaluating 18 kinds of common sample diluent in clinical im- munological test ,and put forward the basic principles to select and estimate clinical immunological sample diluent. Methods The parameters of sample diluents were detected such as blank value,minimum detection limit,interference testing,lin- ear evaluation,and repeatability,then scored all of them synthetically based on the criteria and grading method. Confirmed the diluent with the highest score as the best general one in clinical immunological test. Resalts The special diluent had the highest evaluation score in common sample diluents,followed by the self-made isotonic salt solution containing 4 g/L fetal calf serum with pH 7.4 PBS. Mixed human serum and the diluent without colloidal medium had the lowest scores. The different diluents affected principally the assay sensitivity,rate of recovery and correlation coefficient of the detecting indexes. Conclusion The special diluent had a narrow application rang,while the isotonic salt solution containing 4 g/L fetal calf serum with pH 7.4 PBS could be as the general diluent of high-value sample in clinical immunological lab,which is prepared simply,used easily,also it had a wide range of application.
