急性冠脉综合征合并超重/肥胖者冠状动脉病变特点分析

摘　　要：目的探讨急性冠脉综合征合并超重/肥胖患者冠状动脉病变的特点。方法入选合并超重/肥胖的急性冠脉综合征患者55例,无肥胖的急性冠脉综合征患者38例。所有受试者均进行冠状动脉造影。结果超重/肥胖组冠脉病变以多支病变为主,占52.72%,高于非肥胖组χ,2=11.088,P<0.01。超重/肥胖组冠脉重度病变及完全闭塞分别为27.03%和34.46%,高于非肥胖组,χ2=15.262,P=0.002。超重/肥胖组冠脉血管发生弥漫性病变为52.73%,高于非肥胖组(18.42%)χ,2=11.148,P=0.001。结论急性冠脉综合征合并超重/肥胖患者冠状动脉病变具有多支、弥漫性及狭窄严重的特点。Objective ： To investigate the characteristics of the coronary angiography findings in acute coronary syndrome patients complicated with over-weight or obesity. Methods： Ninety -three acute coronary syndrome patients were enrolled, among whom 55 cases had acute coronary syndrome complicated with over-weight or obesity and 38 cases had complicated without obesity. Coronary angiography was performed in all the patients. Results： Multiple vessel disease was mainly in acute coronary syndrome complicated with over-weight or obesity group （52.72%）, higher than that in acute coronary syndrome complicated without obesity group, χ2 = 11. 088, P 〈 0. 01. In the acute coronary syndrome complicated with overweight or obesity group, the rate of severe stenosis and of complete occlusion of vessel were higher （27.03% and 34.46%, respectively） than those in acute coronary syndrome complicated without obesity group, χ2 = 15. 262, P = 0. 002. And the incidence of diffuse change in acute coronary syndrome complicated with over-weight or obesity （52.73%） was significandy higher than that in acute coronary syndrome complicated without diabetes mellitus group （20.77%） ,χ2 = 11. 148, P = 0. 001. Conclusion： Multiple vessel disease, severe stenosis and complete occlusion of vessel and diffuse change are the characteristics of coronary artery in acute coronary syndrome patients complicated with over-weight Or obesity.
