丙氨酰谷氨酰胺对猪体外循环后肠黏膜屏障的保护作用

摘　　要：目的探讨体外循环丙氨酰谷氨酰胺对猪体外循环后肠黏膜屏障保护作用。方法将16头上海白猪随机分为对照组和实验组,每组8头。实验组在体外循环升主动脉阻断及体外循环60min后开放升主动脉时分别静脉给予N(2)-L-丙氨酰-L-谷氨酰胺0.4g/kg。围手术期检测血浆内毒素、IL-6及TNF-a含量。通过超声检测肠道蠕动情况,评价肠道功能,并对术后肠道进行病理检测。对照组给予同剂量生理盐水,其余步骤同实验组。结果与对照组相比,实验组在体外循环后血浆内毒素、IL-6、TNF-a均降低,超声检查示术后实验组较对照组肠道蠕动增加。术后病理显示,实验组较对照组肠道黏膜屏障破坏明显减轻。结论丙氨酰谷氨酰胺对猪体外循环后肠黏膜屏障有一定的保护作用,并可以指导临床用药。Objective To investigate the effects of N （ 2 ） -L-Alanyle-L-Glutamine （ Gin ） on the intestinal mucosa barrier during cardiopulmonary bypass （CPB）. Methods Sixteen Shanghai white pigs undergoing CPB were randomly divided into two groups： the control group and the experimental group（ n = 8 in each）. Gln at 0.4 g/kg was administrated to the pigs in experimental group at beginning and 60 min after CPB. Plasma endotoxin, IL-6 and TNF-a levels were measured before operation, at the end of CPB and lh after operation. Results Plasma endotoxin, IL-6 and TNF-a levels during CPB and at the end of operation were significantly higher than those of pre-operation in both groups. However, the plasma level of endotoxin, IL-6, TNF-a in experimental group were much lower than those in control group at the postoperative time points. The pathological examination also revealed that the damage of intestinal mucosa was attenuated in experimental group. Conclusion N （2） -L-Alanyle-L-Glutamine can protect the intestinal mucosa barrier in pigs undergoing CPB.
