地特胰岛素替代低精蛋白胰岛素治疗2型糖尿病的疗效评估

摘　　要：目的研究地特胰岛素替代低精蛋白胰岛素(NPH)治疗T2DM的效果。方法用地特胰岛素替代NPH治疗30例T2DM患者,检测HbA_1c、FBG及患者个体FBG变异性,体重变化以及低血糖发生率。检测时间包括在替代前12周,替代后3个阶段(每阶段均为12周)。结果 HbA_1c在第2、3阶段明显改善。体重从62.7 kg降至61.3 kg(第1阶段),FBG明显改善,患者个体FBG变异性比使用NPH时低。结论使用地特胰岛素能良好地控制血糖,短时间体重并未增加。Objective To evaluate the efficacy of detemir insulin therapy replacing neutral protamine Hagedorn（NPH） in patients with type 2 diabetes. Methods Detemir insulin replacing NPH was used in 30 patients with type 2 diabetes and changes in hemoglobin A1C（HbA1c）, fasting blod glucose levels （FBG）, FBG variability, body weight and prevalence in hypoglycemia were monitored and compared among insulin therapies for 12 weeks before and after replacement for three periods（3 × 12 weeks）. Results HbA1 c levels were significantly decreased in periods 2 and 3. Body weight decreased from 62.7kg to 61.3kg in period 1, FBG improved, FBG varibility within-patients was lower in detemir insulin treatment compared with NPH. Conclusions Using detemir insulin to replace NPH can lead to imnrovements in glvcemic control without weight regain.
