移植胚胎数对体外受精-胚胎移植临床结局影响探讨

摘　　要：目的:探讨移植胚胎数对体外受精-胚胎移植(In vitro fertilization and embryos transfer;IVF-ET)临床结局的影响。方法:选择2008年1～12月在中心行IVF-ET或ICSI-ET治疗的1302周期,按移植胚胎数目分为移植1枚组、移植2枚组、移植3枚组,比较3组间的临床妊娠率、胚胎种植率、双胎率、多胎率、流产率、活产率。结果:3组中2枚组的临床妊娠率、胚胎种植率、活产率最高,2枚组和3枚组的临床妊娠率差异无统计学意义(P>0.05),但多胎妊娠率显著低于3枚组(P<0.01),2枚组和3枚组比较有较高的活产率而较低的流产率(P均<0.05)。结论:在IVF-ET周期中,减少移植胚胎数可能会降低临床妊娠率,但移植2枚胚胎能够有效降低三胎以上多胎妊娠的发生。Objective：To explore the effect of transferred embryos number on clinical outcomes of in vitro fertilization-embryo transfer（IVF-ET）.Methods：1 302 IVF-ET or ICSI-ET cycles were selected from January to December in 2008,then they were divided into single embryo transfer group,double embryo transfer group and triple embryo transfer group according to transferred embryos number;the clinical pregnancy rates,implantation rates,twin pregnancy rates,multiple pregnancy rates,abortion rates and live birth rates in the three groups were compared.Results：Among the three groups,the clinical pregnancy rate,implantation rate and live birth rate in double embryo transfer group were the highest,there was no significant difference in the clinical pregnancy rate between double embryo transfer group and triple embryo transfer group（P0.05）,but the multiple pregnancy rate in double embryo transfer group was significantly lower than that in triple embryo transfer group（P0.01）,compared with triple embryo transfer group,the live birth rate in double embryo transfer group was higher,but the abortion rate was lower（P0.05）.Conclusion：In IVF-ET cycle,reducing the number of transferred embryos may reduce clinical pregnancy rate,but transplanting two embryos can reduce the occurrence of multiple pregnancy effectively.
