经颈内静脉入路行左房压监测在复杂先心病矫治术中的临床应用

摘　　要：目的 通过颈内静脉置管途径行持续左房压（LAP）监测,观察其在复杂先天性心脏病矫治术中应用的安全性、准确性及可行性.方法 选择5岁以下复杂先天性心脏病拟行心内直视手术患儿50例,经颈内静脉入路置入18G单腔导管,行持续LAP监测.结果 全组患儿均成功放置左房导管,4例术后回ICU后导管脱落至右房,其余患儿术后均成功行持续LAP监测.本组病例无严重相关并发症发生.结论 经颈内静脉入路行左房压监测在复杂先天性心脏病矫治术中的应用是安全、可行的,并能对复杂先天性心脏病术后患者病情变化的判断及处理提供可靠数据.Objective To investigate the safe and accuracy and practicable route of placing the pipe to monitor the pressure of the left atrium （LAP） through internal jugular vein. Methods 50 children under the age of 5 were placed the singly cavity pipe of 18G to monitor the LAP through Internal jugular vein in the surgical of complex congenital heart disease, and to analyze the LAP＇s waveforms and data. Results All children were successfully placed the pipe to monitor the LAP through Internal jugular vein, 4 cases＇s pipe breaked away from left atrium to right atrium in ICU, no severity complications occurred. Conclusion The way of placing the pipe to monitor the LAP through Internal jugular vein in the surgical of complex congenital heart disease is safe and practicable, and it is able to provide the reliable data to treat the patients of behind the surgical of complex congenital heart disease.
