摘 要:目的探讨24h冰冻血浆(FP24)质量及其制备程序。方法 24份(400ml/份)全血于采后<6h用无菌接驳机一分为二:1份6h内过滤去除白细胞(简称滤白),在其中设对照组[12份滤白后立即制备血浆(FFP)]、实验组1[12份滤白后置4℃过夜(18~24h)后制备血浆(Ⅰ-FP24)];另1份作为实验组2,置4℃过夜后滤白并立即制备血浆(Ⅱ-FP24)。所有血浆分别制备好后立即保存至-30℃冰箱,并于采血后d337℃解冻后做体外质量评估:观察外观、检测FⅤ、FⅦ、FⅧ及Fib,以及主要生化指标含量。结果对照组FFPFib、FⅤ、FⅦ及FⅧ依次为(2.08±0.16)g/L、(86.57±18.73)%、(91.71±25.53)%和(97.65±25.99)%,与之相比,实验组1依次下降22.6%、29.38%、32.89%和40.71%,实验组2相应下降1.9%、-2.09%、14.91%和32.86%。实验组2与对照组的FⅧ分别为(65.56±13.93)%和(97.65±25.99)%(P<0.05),实验组2与实验组1的FⅤ分别为(88.38±15.97)%和(61.14±13.76)%(P<0.05);实验组2的K+、LDH、FHb分别为(4.33±0.28)mmol/L、(145.70±26.00)mmol/L和(129.96±34.33)mg/L,较对照组和实验组1明显增高(P<0.05)。结论全血采后<6h滤白并置4℃过夜再制备的冰冻血浆,凝血因子损失较多;全血采后置4℃过夜(18~24h)后滤白并随后制备的冰冻血浆,则能很大程度地保持凝血因子的效果。Objective To investigate the manufacture procedure and quality of plasma separated from whole blood within 24 hrs of collection.Methods Twenty four bags of 400 ml whole blood(WB,n=24) were collected according to the standard procedures from volunteer donors,and then each bag was divided into two equal units.One unit from each bag was put to group 1 and the other unit to group 2.All units in group 1 were leukodepleted within 6hrs of whole blood collection.Plasma was separated from 12 of group 1 units immediately after leukodepletion(FFP) .The other 12 leukodepleted units of group 1 were stored at 4℃ overnight before separation of plasma(Ⅰ-FP24) .Whole blood units of group 2 were stored overnight at 4℃ and then leukoreduced and plasma separated(Ⅱ-FP24) .All plasma was stored in-30℃ refrigerator before testing.Coagulation factors Fib,FⅤ,FⅦ,and FⅧ,and some biochemical indicators were analyzed.Results The mean levels of Fib,FⅤ,FⅦ,FⅧ in FFP were(2.08±0.16) g/L,(86.57±18.73) %,(91.71±25.53) % and(97.65±25.99) %,respectively,while those values were reduced by 22.6%,29.38%,32.89%,and 40.71% respectively for Ⅰ-FP24,and by 1.9%,-2.09%,14.91%,and 32.86% respectively for Ⅱ-FP24.The levels of FⅧ in FFP and Ⅱ-FP24 were(97.65±25.99) % and(65.56±13.93) %,respectively(P0.05) ,and the levels of FⅤ in Ⅱ-FP24 and Ⅰ-FP24 were(88.38±15.97) % and(61.14±13.76) %,respectively(P0.05) .Compared to FFP and Ⅰ-FP24,Ⅱ-FP24 had higher levels of K+(4.33±0.28 mmol/L) ,LDH(145.70±26.00mmol/L) and FHb(129.96±34.33mg/L) .Conclusion Plasma prepared from WB leukodepleted within 6 hrs,and then stored overnight at 4℃ has much lower levels of coagulation factors.FP24 is best prepared from WB stored overnight at 4℃ and then leukodepleted and plasma separated within 24 hrs.
