摘 要:目的探讨高浓度氯化钾溶液微泵深静脉注射治疗心脏术后严重低钾血症患者的疗效。方法采用高浓度微泵深静脉补钾方法,对85例体外循环心脏术后并发严重低血钾症的患者,根据血清钾水平将10%氯化钾加等量0.9%NS在1~2h之内泵完,在补钾过程中严密进行心电图、血清钾、尿量及肾功能的监护。结果补钾前后血钾浓度比较有显著差异,血钾浓度的改变与补钾速度呈线性关系,补钾2~5h血钾回升到正常者82例(占96.5%),补钾后出现高钾血症者3例(占3.5%),补钾有效率达100%。结论体外循环心脏术后早期低血钾患者,采用高浓度微泵深静脉补钾的方法是安全有效的,既可及时纠正术后低钾血症,而且还可满足心脏术后限制液体入量的要求,提高心脏手术的成功率。Objective To explore the clinical efficacy of postcardiotomy patients with severe hypokalemia undergoing high concentrations of potassium chloride of deep- vein injection by micro-pump. Methods 85 postoperation of extracorporeal circulation patients with severe hypokalemia underwent high concentrations of potassium chloride solution of deep-vein injection by micro-pump. 10% potassium chloride with equal volume normal saline in micro-pump used up in 1 ~ 2 h, and in the whole process ECG, serum potassium, urine output and renal function were monitored. Results There wa significantly different before and after the treatment, a linear relationship between the concentration changes and the rate of potassium pumped. There were 3 hyperkalemia patients (3.5%), and 82 (96.5%) patients' potassium to normal in 2 ~ 5 h. Conclusion High concentrations of potassium chloride of deep-vein injection by micro-pump is a safe and efficient treatment to postoperation of extracorporeal circulation patients with hypokalemia, which can not only correct hypokalemia immediately, but also control liquid input precisely to improve the success rate of cardiac surgery.
