福尔利复合罗库溴铵在高龄患者全麻诱导中的应用

摘　　要：[目的]探讨福尔利复合罗库溴铵用于高龄患者全麻诱导的安全性。[方法]选择90例高龄全麻手术患者随机分为3组。诱导用药:A组静注异丙酚、芬太尼、罗库溴铵,B组静注福尔利、芬太尼、罗库溴铵,C组先预注小剂量罗库溴胺后依次注入福尔利、芬太尼、罗库溴铵。记录麻醉诱导前、气管插管前、插管即刻和插管后3min时患者血压、心率和末梢血氧饱和度值,观察和记录发生肌阵挛等不良反应。[结果]A组气管插管前收缩压明显低于诱导前,差异有统计学意义(P﹤0.05),而B、C组无改变;插管即刻各组心率均较诱导前明显增快(P﹤0.05);B、C组分别有8例和1例出现肌阵挛,两组间差异有统计学意义(P=0.026)。[结论]福尔利联合罗库溴铵作为全麻诱导药对血流动力学影响小,适合高龄患者手术全麻的诱导。[Objective]To discuss the safety of application of etomidate combined with rocuronium bromide in the induction of general anesthesia in geriatrics.[Methods]90 cases of aged patients for general anesthesia were randomly divided into three groups.Inducement of medicine application：Patients in Group A were injected with propofal,fentanyl and rocuronium bromide;those in Group B were injected with etomidate,fentanyl and rocuronium bromide,and patients in Group C were injected with small dosage of rocuronium bromide in advance,then in turn with etomidate,fentanyl and rocuronium bromide.We recorded the blood pressure,heart rate,and peripheral blood oxygen saturation value before inducement of general anesthesia,before trachea intubatton,immediate trachea intubatton and 3 minutes after trachea intubatton;and we observed and recorded myoclonic response and other adverse reaction.[Results]The systolic pressure prior to trachea intubatton in Group A was obviously lower than that before inducement P﹤0.05;while there was no changes in Group B and C.The heart rate in each group at immediate trachea intubatton significantly increased compared to that before inducement（P﹤0.05）.8 cases in Group B and 1 case in Group C appeared myoclonic response.There was significant difference between the two groups（P = 0.026）.[Conclusion]Etomidate combined with rocuronium bromide in the induction of general anesthesia had little affect in hemodynamics,which is suitable for aged patients in the induction of general anesthesia.
