10所医院403例清洁切口手术预防用药调查

摘　　要：[目的]调查成都市10所医院清洁切口围手术期预防使用抗菌药物情况,为合理使用手术预防用药提供依据。[方法]对成都市10所二甲及以上医院所有清洁切口术后住院病人进行调查,术后抗菌药物未停用者1周后追踪完成调查,分析其中清洁切口围手术期预防使用抗菌药物情况。[结果]共调查453例清洁切口术后患者,403例预防使用了抗菌药物;抗菌药物术前使用率59.55%(单联用药79.14%)、术中使用率11.41%(单联用药80.43%)、术后使用率99.50%(双联用药41.40%、三联及以上用药11.72%);只有26.13%的手术持续时间﹥3h患者术中接受了抗菌药物;42.50%的术前用药时间超过24h,92.51%的术后用药时间超过48h。[结论]本次调查揭示了本地区清洁切口围手术期预防抗菌药物使用率高,用药指征不严,用药时机不当,用药疗程长,联合用药不合理,应采取措施加强抗菌药物的合理应用。[Objective]To investigate and analyze the prophylactic usage of antibiotics during perioperative period in clean surgical operation in the hospitals of Chengdu,and to provide reference for rational preoperative use of prophylactic antibiotics.[Methods]All patients who used antibiotics during perioperative period in clear surgical wound operation in the second level hospitals were included in our study,and patients who stopped to use the antibiotics for one week were followed up,and we analyzed the usage of antibiotics for the prevention of clear surgical wound.[Results]453 clean surgical operation cases was investigated,403 cases received prophylactic antibiotics.The using rate was 59.55% pre-operation（79.14% received single drug）,11.41% inter-operation（80.43% received single drug）,99.50% post-operation（41.40% received combination of 2 drugs,11.72% received combination of 3 drugs）,and the operating time of 26.13% inter-operation was longer than 3h.The using time longer than 24h was happened in 42.50% cases received pre-operative drugs,and longer than 48h in 92.51% cases received post-operative drugs.[Conclusion]This investigation revealed that the prophylactic antibiotics using rate was high during preoperative period in clean surgical operation in Chengdu area.The timing,duration and combination of prophylactic antibiotics usage tended to be irrational.Therefore,intervention is urgently needed to tackle this problem.
